Have you ever experienced premature ejaculation? Most men have, at least a few times in their lives — myself included. If it happens rarely, it is not a problem and needs no treatment. But if it happens often enough to trouble you or your partner, the natural question is what to do about it first. This article covers seven things you can try yourself before any prescription is needed.

Premature ejaculation is the most common male sexual disorder.

First, it helps to be clear about what premature ejaculation actually means. Different organisations word it differently, but the core idea is the same: an inability to control ejaculation, which then happens sooner than wanted. Two of the standard definitions are set out below.

Definitions of premature ejaculation

  1. NS 世界保健機関 (WHO) describes PE as an inability to delay ejaculation sufficiently to enjoy lovemaking, shown either by ejaculation before or very soon after intercourse begins, or by ejaculation occurring without a sufficient erection for intercourse to be possible.
  2. NS 米国泌尿器科学会 (AUA) defines it as ejaculation that occurs sooner than desired, either before or shortly after penetration, causing distress to one or both partners.

The distress falls on both partners, not just the patient.

One question to answer before you start: has this always been the case?

This is the most useful question in the whole subject, and it decides whether the self-help below is the right starting point.

If it has been this way since you first became sexually active, the techniques below are exactly where to begin. But if you used to have good control and lost it, something has usually changed, and finding what changed is more productive than practising technique. The commonest explanations are worth knowing:

None of that means self-help is wasted — it helps in either case. It means that if control was lost rather than never gained, an assessment is worth having sooner rather than after months of technique.

Now to the practical part — the seven things you can do yourself.

1. Talk openly with your partner

There is clear evidence that anxiety, negative thoughts and relationship difficulties can bring on premature ejaculation. Talking honestly and understanding each other better genuinely improves sexual performance — this is not a soft option, it is often the single most effective step.

It is worth naming the cycle explicitly, because recognising it is half of breaking it: an episode leads to worry about the next time, that worry raises arousal and self-monitoring, and the heightened state makes another episode more likely. The loop feeds itself, and it is the reason the problem so often gets worse in silence and better once it is spoken about.

2. Notice what makes it better or worse

Pay attention to the circumstances — how tired you were, how long since the last time, how anxious you felt, what medication you had started. Identifying your own pattern tells you what to change, and it is far more useful to bring to an appointment than a general description.

One pattern deserves a warning rather than a note. Alcohol does delay ejaculation for some men, and a number of men work that out for themselves and start relying on it. Please do not build a strategy on it. Alcohol worsens erections at the same time as it delays ejaculation, so it tends to trade one problem for another; the amount needed creeps up; and using a drink to manage a sexual difficulty is a well-worn path to a larger difficulty. If a drink is currently the only thing that works, that is a reason to come and be assessed, not a solution to keep.

3. The “Start-Stop” technique

This technique was first proposed more than 50 years ago and remains useful. It is simple: as ejaculation approaches, ask your partner to stop all genital stimulation until the excited state subsides, then resume. The cycle is repeated as desired, and ejaculatory control gradually improves with practice.

Two things make it work better. It needs your partner’s agreement and involvement rather than being done to them, and it needs practice over weeks rather than a single attempt — most men who conclude it does not work gave it two or three tries. It can also be practised alone at first, which many men find easier before bringing it into partnered sex.

4. The “Squeeze” technique

As ejaculation approaches, the partner firmly squeezes the junction between the glans and the shaft of the penis until the erection partly subsides. As with Start-Stop, the cycle is repeated as desired. Firmly does not mean forcefully — it should not be painful, and pain is a signal to ease off rather than to persist.

5. Exercise

Regular exercise raises serotonin levels, and serotonin is the neurotransmitter that lengthens the time to ejaculation. A randomized trial found that moderate-intensity exercise for more than 30 minutes, at least 5 times a week, significantly improved premature ejaculation.

6. Condoms

A condom reduces sensitivity of the glans. Some manufacturers also add a mild topical agent for the same purpose, which lowers sensation further and lengthens the duration of intercourse.

7. Topical penile agents

Desensitizing sprays or creams containing lidocaine, applied to the tip and shaft of the penis 10–15 minutes before sexual activity, significantly lengthen time to ejaculation. Two instructions must be followed carefully:

Two additions to those instructions. Wearing a condom over the treated area is a simpler way of preventing transfer than wiping, and worth considering if numbness for your partner has been a problem. And do not apply these agents to broken or inflamed skin, or continue using them if the skin becomes irritated or a rash develops — occasional allergic reactions to topical anaesthetics do occur, and that needs a different approach rather than persistence.

Those are the seven self-help measures. If premature ejaculation continues to trouble you after trying them, it is worth discussing with a urologist or andrologist — effective medical treatments exist, and the problem responds well to them. In practice that usually means a tablet from a class of medicines that delays ejaculation, taken either daily or before sexual activity depending on the drug and the pattern of the problem. Several of these are used for this purpose outside their original licence, which is common and legitimate in medicine but is something you should be told rather than discover afterwards. Where erectile difficulty is part of the picture, treating that is generally addressed first or alongside, because the two are so often tangled together.

A word on what is sold for this online, since a great deal is. Sprays, tablets and supplements marketed for lasting longer are frequently of unknown content, occasionally contain prescription drugs that are not declared, and are not a substitute for finding out why the problem is happening. If self-help has not worked, an appointment costs less over time than a cupboard of things that did not.

Q3: I used to have good control and lost it. Does that matter?

It matters a great deal, and it is the most useful question in this subject. Premature ejaculation that has been present since a man first became sexually active is different from control that was lost, because in the second case something has usually changed and finding it is more productive than practising technique. The commonest explanation is erectile difficulty: a man struggling to keep an erection unconsciously hurries, and what looks like premature ejaculation is the erection problem underneath. Treating the ejaculation alone then fails. Other causes include an overactive thyroid, which is uncommon but treatable and found on a simple blood test, prostate inflammation, and newly started medications or substances. If control was lost rather than never gained, seek assessment sooner rather than after months of self-help.

Q4: Does exercise really help premature ejaculation?

はい。無作為化臨床試験では、週5回以上、1回30分以上の適度な強度の運動が、早漏を著しく改善することが示されました。運動はセロトニン神経伝達物質のレベルを上昇させ、射精時間を延長するのに役立ちます。.

Q5: How does the Start-Stop technique work for PE?

The Start-Stop technique involves stopping all genital stimulation when close to ejaculation, waiting until the excited state subsides, then resuming stimulation. This cycle is repeated to gradually improve ejaculatory control over time. It works best with a partner’s agreement and involvement, and it needs practising over weeks rather than a single evening — most men who conclude it does not work tried it two or three times. It can also be practised alone first, which many find easier.

Q6: Does alcohol help premature ejaculation?

It can delay ejaculation for some men, but it is not a strategy worth building on. Alcohol worsens erections at the same time as it delays ejaculation, so it tends to trade one problem for another; the amount needed tends to creep up; and using a drink to manage a sexual difficulty is a well-recognised route to a larger difficulty. If a drink is currently the only thing that works, that is a reason to be assessed rather than a solution to keep.

Q7: Are the sprays and supplements sold online worth trying?

Generally not. Products marketed for lasting longer are frequently of unknown content, occasionally contain undeclared prescription drugs, and do nothing to establish why the problem is happening. Topical anaesthetic agents used properly are a legitimate option, as described above, but should not be applied to broken or inflamed skin and should be stopped if irritation or a rash develops, since allergic reactions to topical anaesthetics do occur. If self-help has not worked, an assessment is a better use of money than accumulating products.

Q8: When should I see a doctor for premature ejaculation?

You should consult a urologist or andrologist if premature ejaculation occurs frequently, causes significant distress, affects your relationship, or does not improve with self-help techniques. Come sooner if control was lost rather than never gained, if your erections are also less reliable, or if there is pelvic pain, painful ejaculation or urinary symptoms alongside. Medical treatments including topical therapies and medications are available and highly effective.

Q9: Can I discuss this by video consultation?

Yes, and many men prefer to start that way with this particular subject. Bangkok Hospital Headquarters offers a Telemedicine service, arranged in advance by email to the Urology department at bhquro@bdms.co.th. The consultation is largely history — how long, whether control was ever good, what your erections are like, what medications you take — and that history is what determines whether any tests are needed. Examination and blood tests are arranged in person afterwards where required. Samitivej Sriracha is in-person only, booked through the Urology department line 088-022-1445.
