If the pain is happening now

Go to an emergency department. Everything on this page is about episodes that have already settled. Pain that is present now, and severe, is treated as перекрут яичка until proved otherwise, and what can be saved is decided in hours. In Thailand, call 1669.

The pattern

Men and boys arrive at the emergency department in extraordinary pain — unable to do anything but lie still. Then, sometimes before anyone has examined them, it stops. The testicle has twisted and untwisted on its own.

That is intermittent testicular torsion, and its whole difficulty lies in the fact that it resolves. By the time the scan is done everything looks normal, everyone relaxes, and the patient goes home — until it happens again, or until the twist that does not undo itself.

What the episodes look like

Most often this happens to adolescents and young men, but not exclusively. Parents of a boy who has had one of these episodes should mention it even if it lasted ten minutes and he now seems perfectly well.

Why the scan does not settle it — in either direction

This is the part worth reading twice, because the logic runs both ways and the earlier version of this article gave only one of them.

Between episodes, a normal Doppler ultrasound is expected and means nothing. The blood supply has been restored because the testicle untwisted. A normal scan in a boy describing this history does not make the history less worrying — it is exactly what the diagnosis predicts.

And during an episode, a normal scan does not exclude torsion either. Flow can still be detected in a testicle that is twisted, particularly where the rotation is partial. A normal ultrasound must therefore never be the reason someone in current severe pain is sent home. Where the story and examination fit, exploration is the answer rather than reassurance.

In both situations the diagnosis rests on what the patient describes, not on the picture. Intermittent torsion is diagnosed by history.

Why this is worth acting on

The anatomy that lets a testicle rotate does not correct itself. Each episode is the same event that, on some occasion, will not undo — and by then the outcome depends on how quickly someone reaches an operating theatre.

Орхидопексия stitches the testicle to the scrotal wall so it cannot rotate. Both sides are fixed, because the anatomy responsible is usually present on both. Done electively it is a short, planned operation with no emergency, no period without blood supply, and no question of whether the testicle survives.

The comparison is stark, and it is the reason this article exists. The same operation, done in advance, saves the testicle with certainty. Done in an emergency, it saves it only if you got there in time.

While waiting for surgery there is nothing you need to avoid — attacks are not reliably brought on by anything you can control. What matters is knowing that an episode which does not settle within a short time is the one to go to hospital for.

How is it different from ordinary testicular torsion?

It is the same event that undoes itself. The testicle twists on the cord, the pain is identical, and then it untwists spontaneously and the pain disappears. Full torsion does not undo, which is why it becomes an emergency. Recognising the intermittent form identifies the same anatomy before it causes permanent harm.

My ultrasound was normal. Does that rule it out?

No, and between episodes a normal scan is exactly what the diagnosis predicts, since the blood supply has been restored. It is diagnosed on the history. Equally, during an episode a normal scan does not exclude torsion either — flow can persist with partial rotation — so a normal ultrasound should never be the reason someone in current severe pain is sent home.

What are the warning signs?

Sudden severe pain in one testicle, at full intensity from the start, often radiating to the groin, settling by itself usually within an hour, and recurring weeks or months later. During the attack the testicle may sit high or lie across rather than up and down.

The pain has gone. Do I still need surgery?

That is the point of recognising it. The anatomy allowing rotation does not correct itself, and the same twist will eventually fail to undo. Fixing both testicles as a planned operation prevents that, and it is a far better operation than the same one performed as an emergency with the testicle already deprived of blood.

Why fix both sides if only one hurts?

Because the anatomical variant that allows a testicle to rotate is usually present on both sides. Fixing only the symptomatic side leaves the other one exposed.
