Varicocele treatment: An oral medication role

As I have previously discussed varicocele disease and its effect on male fertility, most gentlemen who experience this problem ultimately end up with surgical correction. This topic will present an alternative oral medication that may improve symptomatic varicocele treatment.

To understand the treatment rationale, we must first understand how varicocele harms fertility. The testicle functions optimally at around 35°C. Varicocele causes venous backflow of blood at core body temperature (37°C) to pool around the testicle, raising testicular temperature and impairing sperm production, leading to infertility and testicular discomfort.

From literature review, only one scientifically approved oral medication exists for varicocele — Micronized Purified Flavonoid Fraction (MPFF). MPFF improves venous tone and contractility and decreases venous blood pressure. It is also used by vascular surgeons for varicose veins of the lower extremities.

Varicocele treatment: Oral medication strategy

A clinical trial on MPFF published in 2005 showed the following results after 6 months of treatment:

1. Testicular discomfort
   • 87.5% of patients became completely free from testicular discomfort
   • 12.5% experienced significant improvement in testicular discomfort
2. Infertility
   • Significantly improved sperm motility
   • No significant improvement in semen volume, sperm concentration, or morphology

MPFF may be a useful alternative oral treatment for varicocele-related pain in patients who wish to avoid surgery. However, it appears to have limited effect on improving overall sperm quality. Any questions, you can discuss with your trusted andrologist.

Can varicocele be treated without surgery?

Yes, for symptomatic varicocele (pain and discomfort), oral medication with Micronized Purified Flavonoid Fraction (MPFF) has shown clinical benefit. In a clinical trial, 87.5% of patients became completely free from testicular discomfort and 12.5% showed improvement after 6 months of MPFF. However, surgical correction (varicocelectomy) remains the primary and most effective treatment, especially when fertility improvement is the goal.

Does MPFF improve fertility in varicocele patients?

MPFF shows a significant improvement in sperm motility after 6 months of treatment. However, it does not improve semen volume, sperm concentration, or morphology. For patients whose primary concern is fertility rather than pain, surgical varicocelectomy remains the more effective option, as it directly addresses the venous backflow and temperature elevation that impair sperm production.

How does varicocele cause infertility?

The testicle produces healthy sperm at around 35°C, slightly below core body temperature. Varicocele causes abnormal venous backflow of blood at 37°C to pool around the testicle, raising its temperature and impairing spermatogenesis. This leads to reduced sperm quality and male infertility. Correcting the blood flow — either surgically or with venous tone agents like MPFF — is the basis of varicocele treatment.
