Testosterone replacement therapy in male octogenarians: clinical outcomes and adverse event rates

What the study asked: Men aged 80 and over are almost entirely absent from the trials that guide testosterone prescribing — both TRAVERSE and the T-Trials excluded or did not report separately on them, and both excluded men with a history of prostate cancer. This single-centre retrospective cohort followed 46 hypogonadal men aged 80 or older through 36 months of treatment.

What it found: Monitoring findings were reassuring. No patient reached a haematocrit of 54% or above, the threshold that usually forces treatment to change, and there was no biopsy-confirmed prostate cancer recurrence — notable because 39% of the cohort had a prostate cancer history. Blood pressure changed modestly overall (median +3 mmHg systolic), though six men rose by at least 20 mmHg, which is why blood pressure and haematocrit monitoring remain necessary rather than optional in this age group. All-cause mortality was 8.7% and 28.3% stopped treatment over the three years.

What it cannot show: The authors are explicit about the limits. There was no untreated comparison group, so the cardiovascular and thromboembolic events cannot be attributed to the treatment rather than to a medically complex population — 60.9% had hypertension and 30.4% coronary artery disease at baseline. The sample is small, the setting is a single centre, and the retrospective design means events treated elsewhere may have been missed. The paper calls for larger prospective studies with age-matched controls.
