If you have recently undergone an ESWL procedure, or are considering it for your kidney stone, this article will give you an honest picture of what to expect. ESWL (Extracorporeal Shockwave Lithotripsy) is the least invasive kidney stone treatment available. Immediate post-ESWL side effects
- Blood in urine (hematuria) — almost 100% of patients
- ESWL uses focused shockwave pulses to fragment kidney stones. Minor kidney tissue injury from stone debris is unavoidable, resulting in temporary blood in urine
- Blood and stone debris in urine are expected after every ESWL procedure
- This typically resolves within a few days
- Drinking 2–3 litres of water daily significantly speeds up recovery of hematuria

- Steinstrasse (stone street) — 4–7% risk
- A German term meaning stone street — crushed stone fragments can accumulate in the ureter, forming a visible column on imaging
- May cause flank pain or may be asymptomatic; monitoring and, if needed, further intervention is arranged
- Residual stone regrowth — 21–59% risk
- Residual fragments under 4 mm are considered clinically insignificant and may pass spontaneously
- 21% chance of insignificant fragments growing at 5 years — requires regular ultrasound follow-up

- Stone pain (renal colic) — 2–4% risk
- Crushed stone fragments passing through the ureter can cause sudden flank pain
- Drinking 2–3 litres of water daily reduces this risk; oral medications are also available to help reduce stone colic after ESWL

- Bloodstream infection (sepsis) — 1–2.7% risk
- Bacteria can reside within kidney stones. When the stone is fragmented, bacteria may enter the bloodstream, causing sepsis
- Routine oral antibiotics after ESWL are not recommended (to avoid antibiotic resistance); instead, the patient is monitored for fever in the days following the procedure. Fever after ESWL requires prompt medical evaluation
- Significant kidney bleeding — less than 1% risk
- ESWL is the least invasive kidney stone procedure; severe kidney injury is very rarely reported
Long-term effects of ESWL
There is no existing evidence in the medical literature supporting long-term complications from ESWL. The European Association of Urology 2020 guidelines confirm no long-term adverse effects have been demonstrated.

Frequently Asked Questions About Post-ESWL Care
Is blood in urine normal after ESWL?
Yes — blood in urine (hematuria) is expected in almost all patients after ESWL and is not a cause for alarm. The shockwaves that break the kidney stone inevitably cause minor injury to surrounding kidney tissue, resulting in temporary bloody urine that also contains stone debris. This resolves naturally within a few days. The most important thing you can do is stay well-hydrated — drink at least 2–3 litres of water per day. Seek medical attention if hematuria is very heavy, clotted, or persists beyond one week.
What should I watch for after ESWL that requires urgent care?
The main warning signs that require prompt medical evaluation after ESWL are: (1) high fever or chills — may indicate bloodstream infection (sepsis) from bacteria released when the stone is fragmented; (2) severe or worsening flank pain that does not improve — may indicate steinstrasse or significant stone colic; and (3) inability to urinate. Minor discomfort, mild bloody urine, and passing small stone fragments are all expected and normal. If you develop fever after ESWL, contact your urologist or go to the emergency department promptly.
Do I need follow-up after ESWL and how long does recovery take?
Yes — follow-up is essential. Stone clearance is confirmed by ultrasound or KUB X-ray at 4–6 weeks after ESWL. Residual stone fragments smaller than 4 mm may pass spontaneously but carry a 21% risk of regrowth at 5 years — requiring regular ultrasound follow-up. Most patients return to normal activity within 24–48 hours. Continue drinking 2–3 litres of water daily throughout recovery. If you experience significant pain, fever, or prolonged bloody urine, contact your urologist.
