A prostate biopsy is over in minutes, and then you go home and start noticing things. Most of what you notice is expected. One thing is not, and it is worth knowing the difference before it happens rather than at eleven o’clock at night.

The one thing that cannot wait
Fever or shaking chills after a prostate biopsy means go to hospital the same day. Not the following morning, not after seeing whether it settles. Infection introduced during a biopsy can move from feeling like flu to being seriously unwell within hours, and it is treated in hospital rather than with a tablet at home.
This is the one instruction that matters more than everything else on this page. In an emergency in Thailand, call 1669.

What is normal afterwards
- มีเลือดปนในปัสสาวะ Very common — the majority of men. Usually light and settling over several days. Drinking plenty helps.
- Blood in the semen. Common, and this is the one that catches men out. It can persist for several weeks, and the colour can be dark brown or rust rather than red. An earlier version of this article said all bleeding settles within ten days, which is accurate for urine and misleading for semen — men have been alarmed by this at week four believing something had gone wrong. It has not.
- Blood from the back passage, if the biopsy was taken through the rectum. Usually small and short-lived.
- Soreness and a dull ache low down or in the perineum for a few days.
- Stinging on passing urine and going more often for a day or two.
- Feeling faint or sweaty during or just after the procedure. A common vasovagal reaction, not a complication, and it passes with lying down.
On the percentages that used to be listed here
This article previously carried a detailed table of complication rates to one decimal place, including separate entries for persistent painful urination และ pain or burning on urination, which describe the same thing at rates that contradicted each other, and an entry for heart attack at 0.3%. That last figure reflects what happens to a large group of older men over the following weeks; it is not a complication of the biopsy, and listing it as one was misleading.
The precision was also false — figures like this vary widely between series and by technique. What a man actually needs is not a decimal place but the shape of it: bleeding is common and expected, serious infection is uncommon but real and is the thing to act on.
อาการที่ต้องรีบพบแพทย์ภายในวันเดียวกัน
- มีไข้หรือหนาวสั่น The most important one, at any point in the first week or two.
- Feeling faint, confused or profoundly unwell, particularly with a fever.
- ปัสสาวะไม่ออก มีถุงน้ำบิ่วเต็มปัสสาวะอย่างเจ็บปวด.
- Heavy bleeding or clots — as opposed to lightly stained urine — or bleeding from the back passage that does not stop.
- Pain that is increasing after the first couple of days rather than settling.
- Burning and frequency getting worse rather than better after two or three days.
Anything else — including how soon to return to work, exercise, sex or a blood-thinning medicine, and when your result will be ready — should come from the team who performed your biopsy, since it depends on how many samples were taken and what else you are being treated for. Ask before you leave, and take the written instructions with you.

Why the route your biopsy took matters
The infection risk described above belongs almost entirely to the transrectal route, in which the needle passes through the wall of the rectum and carries bowel bacteria with it. No amount of rectal preparation removes that, and antibiotic prophylaxis before the procedure reduces it without abolishing it — increasingly so, as resistant organisms become commoner.
An earlier version of this article described transrectal biopsy as the gold standard. That is no longer accurate and has been corrected. Practice has moved towards the transperineal route, in which the needle passes through the skin instead, avoiding the rectum entirely and making serious infection much less likely. It contradicted the account given elsewhere on this site, and the two now agree.
The full comparison — and why MRI targeting and the choice of route are two separate things that are constantly confused — is set out under การตัดชิ้นเนื้อต่อมลูกหมากผ่านผิวหนังบริเวณฝีเย็บด้วยเทคนิค MRI-fusion.

Waiting for the result
The wait is the hardest part for most men, and two things are worth holding on to while it passes.
A negative result does not entirely close the question — a biopsy samples part of the gland, so if the PSA keeps rising or the MRI stays suspicious, the matter is followed up rather than dropped. And a positive result does not automatically mean an operation: some prostate cancers grow slowly enough that monitoring is the correct management, and what the pathologist reports about grade drives what happens next far more than the word cancer does.
If treatment is needed, see the surgical options and how candidates are assessed.
คำถามที่พบบ่อย
Is blood in the urine, semen or stool normal after a prostate biopsy?
Yes, all three are expected. Blood in the urine is very common and usually settles over several days. Blood from the back passage, after a transrectal biopsy, is usually small and brief. Blood in the semen can last several weeks and often looks dark brown rather than red — this is the one men most often mistake for a complication, and it is not.
What are the warning signs of infection?
Fever or shaking chills above all, and feeling faint, confused or profoundly unwell. These need hospital assessment the same day rather than a clinic appointment, because infection after biopsy can worsen within hours. Difficulty passing urine, heavy bleeding with clots, and pain that increases rather than settles also need urgent review.
Is transrectal biopsy still the gold standard?
No, and an earlier version of this article said it was. Practice has moved towards the transperineal route, which passes the needle through the skin rather than the rectal wall and makes serious infection considerably less likely. Antibiotics before a transrectal biopsy reduce that risk but do not remove it.
How long before I can go back to normal activity?
That depends on how many samples were taken, the route used, and what medication you take, so it comes from the team who performed the biopsy rather than from a general page. Ask before you leave and take the written instructions home with you.
My biopsy was negative. Does that settle it?
Not entirely. A biopsy samples part of the gland rather than all of it. If the PSA continues to rise or the MRI remains suspicious, the question is followed up rather than considered closed.
