Today we are going to talk about MRI of the prostate. As I have often explained when discussing prostate cancer, this disease is frequently limited in aggressiveness. The story usually begins with a rising Prostate-Specific Antigen (PSA) blood test from a routine health check-up, which brings the patient to a urology clinic to weigh the pros and cons of a tissue biopsy and, ultimately, to reach a diagnosis. The important question is this: does every man with a suspicious PSA really need a biopsy? The answer is very often no — and here is the evidence behind that.

Autopsy studies of men who died of unrelated causes have long shown that a substantial proportion of older men are walking around with prostate cancer that never troubled them in life — and that a large share of those cancers are low-grade disease that would never have required treatment. This matters because a needle biopsy of the prostate is not a trivial procedure. It can cause blood in the urine, blood in the stool, blood in the semen, and — uncommonly but importantly — a serious bloodstream infection. In the PROMIS study, close to 6% of participants reported a serious adverse event, including eight cases of sepsis.

When a biopsy is genuinely necessary, it should absolutely be done. The real challenge is knowing how to avoid the unnecessary ones. This is where MRI of the prostate has become an essential tool for urologists. Here are its main advantages and limitations.

What is an MRI prostate used for?

An MRI prostate is a non-invasive imaging tool used to evaluate the prostate gland in men with elevated PSA levels. It helps urologists identify suspicious areas that may indicate aggressive prostate cancer and decide whether a biopsy is necessary, reducing unnecessary invasive procedures.

How accurate is MRI prostate in detecting cancer?

In the PROMIS study, multiparametric MRI detected 93% of clinically significant prostate cancers, compared with 48% for standard transrectal ultrasound-guided biopsy, with a negative predictive value of 89%. Its weakness is specificity, which was 41%, so a suspicious scan still needs biopsy confirmation. In the PRECISION trial, MRI-targeted biopsy diagnosed clinically significant cancer in 38% of men versus 26% with standard biopsy.

Can MRI prostate replace prostate biopsy?

No, MRI prostate cannot fully replace biopsy. If MRI shows suspicious findings, a biopsy is still required to confirm the diagnosis. However, MRI can spare a large number of men the procedure entirely: PROMIS estimated that around 27% of men could avoid a primary biopsy, and in PRECISION 28% of men in the MRI arm had a non-suspicious scan and were not biopsied.

What are the disadvantages of MRI prostate?

MRI prostate is more expensive than standard ultrasound, and it has limited specificity — 41% in the PROMIS study — so abnormal scans often turn out not to be significant cancer. In some cases an endorectal coil must be inserted into the rectum to improve image quality, which can be uncomfortable. It is also not a definitive diagnostic test, so a biopsy is still needed when findings are suspicious, and a small number of significant cancers can be missed.

Who should consider getting an MRI prostate?

Men with elevated PSA levels, an abnormal digital rectal examination, or a previous negative biopsy but ongoing clinical suspicion for prostate cancer are the best candidates for MRI prostate. Your urologist will assess your individual situation and recommend the most appropriate imaging strategy.
