Today we are going to talk about MRI of the prostate. As I have often explained when discussing prostate cancer, this disease is frequently limited in aggressiveness. The story usually begins with a rising Prostate-Specific Antigen (PSA) blood test from a routine health check-up, which brings the patient to a urology clinic to weigh the pros and cons of a tissue biopsy and, ultimately, to reach a diagnosis. The important question is this: does every man with a suspicious PSA really need a biopsy? The answer is very often no — and here is the evidence behind that.

Autopsy studies of men who died of unrelated causes have long shown that a substantial proportion of older men are walking around with prostate cancer that never troubled them in life — and that a large share of those cancers are low-grade disease that would never have required treatment. In other words, finding a cancer is not the same as finding a cancer worth treating.

This matters because a needle biopsy of the prostate is not a trivial procedure. It can cause blood in the urine, blood in the stool, blood in the semen, and — uncommonly but importantly — a serious bloodstream infection. In the PROMIS study, close to 6% of participants reported a serious adverse event, including eight cases of sepsis.
When a biopsy is genuinely necessary, it should absolutely be done. The real challenge is knowing how to avoid the unnecessary ones. This is where MRI of the prostate has become an essential tool for urologists. Here are its main advantages and limitations.
Advantages of prostate MRI
- It is a non-invasive imaging test that provides a great deal of detail about the anatomy of the prostate gland.
- It is far more sensitive than a blind biopsy. In the PROMIS study, multiparametric MRI detected 93% of clinically significant cancers, compared with just 48% for standard transrectal ultrasound-guided biopsy.
- A negative scan is reassuring. PROMIS reported a negative predictive value of 89% for clinically significant cancer, which is what makes MRI usable as a triage test before biopsy.
- It lets many men skip the biopsy altogether. PROMIS estimated that using MRI to triage would allow about 27% of men to avoid a primary biopsy; in the PRECISION trial, 28% of men in the MRI arm had a scan that was not suspicious and were not biopsied at all.
- When a biopsy is needed, targeting finds more of the cancers that matter. In PRECISION, MRI-targeted biopsy diagnosed clinically significant cancer in 38% of men versus 26% with standard biopsy — using a median of only 4 cores instead of 12.
- It reduces overdiagnosis. PRECISION also found 13 percentage points fewer diagnoses of clinically insignificant cancer in the MRI-targeted group — fewer men labelled with a cancer that was never going to harm them.

Limitations of prostate MRI
- It is expensive compared with conventional ultrasound.
- It is not very specific. In PROMIS, specificity was only 41% and the positive predictive value 51% — meaning a suspicious-looking area is roughly a coin flip, and a scan that looks abnormal still has to be confirmed by biopsy.
- Some scanners require an endorectal coil to be placed in the rectum to improve image quality, which many men find uncomfortable.
- A negative scan is reassuring, not a guarantee. A small number of significant cancers are still missed, which is why men with a negative MRI need continued PSA follow-up rather than simple discharge.
