The emergency number in Thailand is 1669. The following need a hospital emergency department the same day rather than a clinic appointment or a telemedicine consultation. An erection lasting more than four hours, particularly a painful one occurring without sexual arousal, is a urological emergency called priapism. Treatment within the first four to six hours usually resolves it with full recovery, while delay beyond twenty-four hours makes permanent erectile dysfunction likely. This applies to any man using intracavernosal injection therapy for erectile dysfunction, and to men with thalassaemia, sickle cell disease or a related haemoglobin disorder, who are at higher risk. Also urgent: testicular pain of sudden onset, which may be torsion and can cost the testicle within hours; complete inability to pass urine; fever with flank pain, which in the presence of a kidney stone means an obstructed and infected kidney and requires urgent drainage rather than antibiotics alone; and a hot, red, swollen scrotum with fever. In a boy or a man, a foreskin that has been pulled back and will not return, with the glans becoming swollen or dark, is paraphimosis and needs assessment the same day at any age. Genital or perineal pain that is out of proportion to how the skin looks, with redness spreading over hours, purple, grey or black discolouration, a crackling feeling under the skin, or fever and severe illness, may be a rapidly spreading deep tissue infection; it is uncommon, it is life-threatening, and men with diabetes are at greatest risk. In a man known to have a kidney angiomyolipoma, sudden severe flank or abdominal pain with faintness, pallor, sweating or a racing pulse suggests bleeding from the tumour. In a child, frequent urination together with constant thirst, weight loss, tiredness or a return to bedwetting after a long dry period needs a same-day blood sugar test, and if there is also abdominal pain, vomiting, deep or rapid breathing or drowsiness it is a diabetic emergency. A patient with a spinal cord injury who develops a sudden pounding headache with sweating or flushing above the level of the injury, particularly when the bladder is full or not draining, may have autonomic dysreflexia, in which draining the bladder often relieves it and blood pressure can rise dangerously fast. A patient on testosterone therapy with chest pain, sudden breathlessness, pain or swelling in one calf, or a sudden severe headache with weakness or difficulty speaking should attend hospital and say that he is on testosterone. A patient on potassium citrate who develops muscle weakness, tingling, palpitations, confusion or faintness should stop the medication and be seen the same day, as these can indicate a high blood potassium level. A patient with syphilis or a positive syphilis test who develops any change in vision, new eye pain, new hearing loss, vertigo, severe headache with neck stiffness, or new weakness or confusion needs same-day assessment, since eyesight can be lost and the treatment differs. Post-operative bleeding that soaks a dressing or continues after ten minutes of firm pressure, and fever after any urological procedure including shockwave lithotripsy or prostate biopsy, are also same-day matters.
