With a stapler circumcision, the aftercare matters as much as the operation. This page sets out the instructions given to Dr. Soarawee’s patients following the procedure.
If you were circumcised elsewhere, follow the instructions your own surgeon gave you — devices and timings differ. Use this page for orientation and for the warning signs below, which apply whoever operated. This page describes aftercare for an adult. If you are caring for a child, follow the instructions given for that child rather than these — the device, the dressing and the timings differ, and several sections below do not apply at all. The warning signs still do, and in a young child they are harder to spot: watch for a child who will not settle, who is crying on passing urine, who is off his feeds or feverish, or whose bandage looks tighter or more soaked than when you left the hospital.

When to get help rather than wait
This section was missing from the earlier version of this page and belongs before the routine instructions. Go back to the hospital the same day for any of the following. In an emergency in Thailand, call 1669.
- Bleeding that soaks through the bandage, or that continues after firm pressure has been held for ten minutes. Stapler circumcision relies on the compression bandage in the first days, and this is the commonest reason for an unplanned return.
- Inability to pass urine, or a bandage so tight that passing urine becomes difficult or the swelling increases below it. Do not leave a bandage that feels progressively tighter — go and have it checked rather than removing it yourself.
- The head of the penis turning dusky, dark, cold or increasingly numb beyond the first day.
- Fever or shaking chills.
- Spreading redness up the shaft, increasing swelling after the first couple of days, or an offensive discharge — as opposed to the ordinary swelling and bruising that settle steadily.
- Pain that is worsening after the first two or three days, or a staple that has caught, is digging in, or has partly lifted and is snagging.

Immediately after the operation
- The wound will be covered with a compression bandage to control bleeding.
- You can pass urine normally — the bandage does not block the urinary passage.
The first hour
- The penis still feels numb from the nerve block.
- If there is no immediate complication, you will be discharged from hospital.
After the first hour
- Discomfort begins as the anaesthetic wears off. Take painkillers as prescribed rather than waiting for the pain to build.
The first 2 days
- Keep the compression bandage in place. Return to hospital daily for bandage changes until no minor bleeding is detected.

The first week

- Apply Terramycin antiseptic ointment to the wound twice daily after showering.
- Morning erections may be uncomfortable as they stretch the healing tissue. This is normal. Contact the hospital if erections are excessively painful, since medication can be prescribed.
- No sexual activity. No heavy exercise — no jogging or cycling. Walking is fine.

Weeks 2 to 4
- Terramycin ointment is no longer needed, as the skin has naturally sealed.
- Around 20% of patients notice the staples coming away by day 10, and 95% by day 30. They shed gradually and at different times around the circumference — a few coming away while others remain is the normal pattern rather than a problem.

- Still no sexual activity — the wound is sealed but not yet strong enough. This is the point at which men most often come to grief, and patience here protects the result.

4周后
- If all the staples have come away, you are free to resume all activities.
- If any remain, return for a check-up — they can be removed by hand. This is a routine appointment rather than a complication.
- Residual firmness and mild swelling at the scar line are common at this stage and continue to settle over the following months, so judge the cosmetic result later rather than at four weeks.

How long does recovery take?
Typically four to six weeks. Walking from day one, with activity restricted otherwise during the first week; a gradual return to exercise in weeks two to four; and full activity including sexual intercourse only once the staples have come away, which is the case in 95% of men by day 30.
When do the staples fall off?
They shed naturally as the wound heals — about 20% of men notice them coming away by day 10 and 95% by day 30. They come away at different times around the circumference rather than all at once, which is normal. Any still in place beyond four weeks can be removed by hand at a follow-up appointment.
Can I shower?
Yes. Shower normally but keep the wound away from direct water and soap during the first week, and apply Terramycin antiseptic ointment twice daily afterwards. From week two the ointment is no longer needed, as the wound has sealed.
When can I resume sexual activity?
Not before four weeks. The wound looks sealed well before the tissue is strong enough for the mechanical stress involved. Your urologist will confirm at the follow-up appointment based on healing and whether the staples have come away.
Are morning erections normal?
Yes, and expected. They stretch the healing tissue and may be uncomfortable, usually manageable without medication. If they are excessively painful, contact the hospital — medication can be prescribed to reduce night-time erections during healing.
What is not normal, and what should I do?
Bleeding soaking the bandage or continuing after ten minutes of firm pressure, inability to pass urine, a bandage that feels progressively tighter with increasing swelling, the head of the penis turning dark or cold, fever or chills, spreading redness or offensive discharge, worsening pain, or a staple digging in or snagging.
