Gonorrhea is one of the most common concerns I see in my clinic. The typical patient is a young, sexually active man who develops a purulent (pus-like) discharge from the urethra together with painful urination, usually within 14 days of exposure to an infected partner. In this article I’ll walk you through what you need to know about this sexually transmitted infection, caused by the bacterium Neisseria gonorrhoeae.

Neisseria gonorrhoeae infection
Neisseria gonorrhoeae is a human pathogen transmitted through sexual contact. The World Health Organization estimates that roughly 106 million new cases of gonorrhea occur worldwide every year. More cases are reported in men, largely because men are far more likely to develop noticeable symptoms, and the incidence is higher among men who have sex with men (MSM).

Here is what makes gonorrhea so easy to pass on: more than 50% of infected women never develop any symptoms. When symptoms do appear, they can be subtle — vaginal discharge, painful urination, or pelvic pain. In men, by contrast, over 90% become symptomatic once infected. Gonorrhea is primarily a clinical diagnosis, so confirmatory testing is optional in many cases. That said, the nucleic acid amplification test (NAAT) detects the infection with over 95% accuracy from a simple first-catch urine sample.

Once gonorrhea is diagnosed, dual therapy is recommended, because gonorrhea is very often accompanied by a Chlamydia co-infection. The standard course is a single intramuscular antibiotic injection plus a course of oral antibiotics. Equally important: if a partner may have been exposed, they need the same treatment regimen. Otherwise the infection simply passes back and forth between you.

Patients often ask what happens if gonorrhea is left untreated. The honest answer is that it can cause serious harm to the reproductive health of both men and women — chronic, unexplained pelvic pain, infertility, and pregnancy loss, among other complications.
The good news is that gonorrhea is very straightforward to treat. Even so, it is important to follow a few instructions after treatment:
- Abstain from sex for at least one week while completing the treatment course.
- Practise safer sex — in particular, use a condom with any new or casual partner. One report found that 27% of newly diagnosed HIV patients had a history of gonorrhea within the preceding 12 months.
