What is the current first-line treatment for gonorrhea?

According to the CDC 2020 guideline, the first-line treatment for uncomplicated gonococcal infection of the cervix, urethra, or rectum is a single intramuscular dose of Ceftriaxone — 500 mg for patients under 150 kg and 1 gram for patients over 150 kg. This injection-based regimen replaced previous dual-therapy approaches due to increasing antibiotic resistance in Neisseria gonorrhoeae.

What if I am allergic to Ceftriaxone or it fails to treat gonorrhea?

Alternative regimens include Gentamicin 240 mg IM + Azithromycin 2 grams orally as single doses, or Cefixime 800 mg orally as a single dose. In the rare event of confirmed Ceftriaxone treatment failure, a culture and sensitivity test is strongly recommended to guide the selection of an effective alternative antibiotic, as treatment-resistant gonorrhea strains are emerging globally.

Should Chlamydia be treated at the same time as gonorrhea?

Gonorrhea and Chlamydia frequently co-occur as sexually transmitted infections. When Chlamydia co-infection cannot be excluded based on clinical assessment or testing, Doxycycline 100 mg orally twice daily for 7 days is added to the gonorrhea treatment regimen. Treating both simultaneously prevents under-treatment and reduces the risk of ongoing transmission and complications such as pelvic inflammatory disease.
