Water vapour therapy is a day-case treatment for an enlarged prostate, taking under ten minutes. This page covers what happens afterwards — the part that determines how the next few weeks feel.

When to get help rather than wait

This section was missing from the earlier version of this page and belongs before the routine instructions. You will be at home with a catheter, and some of these cannot wait for the next clinic appointment. Go back to the hospital the same day for any of the following. In an emergency in Thailand, call 1669.

• The catheter stops draining — no urine entering the bag for several hours, particularly with a full, uncomfortable lower abdomen. A blocked catheter needs dealing with, not waiting on.
• The catheter falls out before the planned removal date. Do not attempt to replace it yourself.
• Fever or shaking chills.
• Heavy bleeding, or clots — as opposed to the lightly blood-stained urine that is expected.
• Inability to pass urine after the catheter has been removed, with a painful full bladder.
• Severe or worsening pain, rather than the discomfort that eases day by day.

Right after the procedure

• You will be woken by the anaesthetist after moderate sedation.
• A urethral catheter will be left in place for 1–2 weeks, depending on prostate size.
• After 1–2 hours of observation in the recovery room, the doctor will decide whether you can be discharged or need to stay overnight.

The first 7 days

• 遵医嘱服药。.
• Use painkillers as needed, per the prescription.

While the catheter is in

• 随时携带尿袋，装满时及时排空。.
• There are two bag options: a 2 L overnight bag, or a 500 mL leg bag for an active day. The nurse will show you how to switch between them.
• Keep the bag below the level of the bladder so it drains, and check periodically that urine is actually flowing into it rather than only when it feels uncomfortable.

The day the catheter comes out

• You will be given an appointment at the clinic for removal.
• After removal, 6 hours of observation is required to see whether you can pass urine.
• If you pass urine successfully at least once, you may go home.
• If you cannot pass urine after 6 hours, the doctor will reassess and decide the next step. This happens to a proportion of men and is not a sign that the procedure has failed.

The first 30 days

• 继续口服前列腺药物，直至过渡期结束。.
• Expect some improvement at 2 weeks after the procedure, with the best outcome at 3 months.
• Burning, urgency and going more frequently in the early weeks are part of the expected course rather than a sign something has gone wrong. They settle as the treated tissue is reabsorbed.

Blood in the semen deserves a note of its own, because it alarms men who were not warned: it is expected, it can look dramatic, and it may persist for several weeks. It is not a sign that anything has gone wrong.

How long do I need the catheter?

Most men need a urethral catheter for 1–2 weeks, and the duration depends on prostate size — a larger prostate may need longer. Your urologist will arrange the removal appointment and check that you can pass urine before you go home.

When will my symptoms improve?

Some improvement usually starts around 2 weeks as the initial swelling settles, with the best result at about 3 months once the treated tissue has been reabsorbed. Irritative symptoms in the early weeks are part of that course rather than a setback.

What side effects should I expect?

Commonly painful urination, blood in the urine, urinary retention, urgency and frequency, blood in the semen, urinary infection and pelvic pain — most settling within about three weeks. Blood in the semen can last several weeks and is not a complication.

Can I go home the same day?

Usually yes. After 1–2 hours of observation most men are discharged the same day, with the catheter in place and after a nurse has shown you how to manage the bag. Overnight admission is sometimes recommended.

Do I still need my prostate medication?

Yes, for the first 30 days, to manage symptoms during the transitional healing period. As the treated tissue is reabsorbed and symptoms improve, your urologist will reassess whether it is still needed.

My catheter has stopped draining. What do I do?

Go to the hospital the same day, particularly if your lower abdomen feels full and uncomfortable. A blocked catheter is not something to wait out, and it should not be flushed or replaced at home. The same applies if the catheter falls out before its removal date.
