A weak stream is one of the hardest complaints to judge, because how a man feels about his flow and how his flow actually measures are only loosely related. Young men arrive convinced something is badly wrong and turn out to have entirely normal flows. Older men mention nothing, having adjusted over years, and measure poorly enough to be heading towards retention.
Uroflowmetry replaces the impression with a measurement. It is simple, takes no preparation beyond a full bladder, and involves nothing more than passing urine privately into a funnel. It is also more limited than most people are told, and this article covers both halves of that.

The rule that decides whether your result means anything
This was missing from the earlier version of this article, and it matters more than any of the numbers below.
A flow test is only interpretable if you passed enough urine — as a general rule at least 150 mL. Below that, the bladder never gets going, the flow rate comes out low, and the result looks abnormal in someone whose bladder is perfectly fine. A great many men have been told they have a poor flow on the basis of a 70 mL void.
The opposite error exists too: a grossly overfull bladder that has been stretched produces a falsely low flow. So the instruction is to arrive with a comfortably full bladder, not a desperate one, and to say so if you could not manage a normal amount — the test can simply be repeated.
For the same reason, a single flow study is weak evidence. Flow varies between occasions in the same person, so two studies on different visits give a much fairer picture than one.
What the test measures
- Maximum flow rate (Qmax), in millilitres per second — the headline number.

- Voided volume — the figure that determines whether the rest is valid.
- Voiding time and time to peak flow. A stream that takes a long time to build up suggests the bladder is working hard to overcome something.
- The shape of the trace, which a urologist reads as carefully as the numbers. A smooth bell shape, a long flat plateau, or an interrupted saw-tooth pattern point in quite different directions.
It is almost always paired with an ultrasound scan of the bladder immediately afterwards to measure what is left behind. The flow rate and the residual volume together say considerably more than either alone.
Rough guides for the flow rate
These are conventions rather than diagnostic thresholds, and they assume an adequate voided volume.
- In men, a Qmax above about 15 mL/sec is reassuring and below about 10 mL/sec is likely to be significant. Between the two is a grey zone that is interpreted alongside symptoms, residual volume and prostate size rather than on its own. Flow also declines gradually with age in men with no disease at all.
- In women, flow rates are normally considerably higher than in men. The earlier version of this article gave a normal range of 20 to 36 mL/sec, which reads as though a faster flow were abnormal; it is not. In women the shape of the trace and the residual volume usually carry more information than the peak figure.


The limitation nobody mentions: a low flow does not tell you why
This is the most important thing to understand about the test, and the earlier version of this article did not say it.
A weak flow has two quite different explanations, and uroflowmetry alone cannot distinguish between them:
- The outlet is obstructed — an enlarged prostate or a urethral stricture — and the bladder is pushing hard against resistance.
- The bladder itself is weak — an underactive detrusor — and is not generating enough pressure, with nothing blocking the way at all.
Both produce a low number on the same printout. Only a pressure-flow urodynamic study, which measures bladder pressure while you void, separates them reliably — and the distinction is not academic. Operating on the prostate of a man whose real problem is a weak bladder does not fix his symptoms, which is exactly why assessment before any prostate procedure matters more than the choice of technology. Uroflowmetry is a screening and monitoring tool. It tells you that the flow is poor, not why.
When it is used
- Assessing a weak stream in men — most often benign prostatic enlargement หรือ urethral stricture, which the shape of the trace can help separate.

- Neurological conditions affecting bladder function.
- Measuring the result of treatment — arguably its best use. Comparing a flow before and after treatment is far more meaningful than a single reading in isolation.

Symptoms that need attention rather than a flow test

- Complete inability to pass urine with a painful full bladder — acute retention, needing treatment the same day.
- Visible blood in the urine, with or without clots.
- Fever with flank or lower abdominal pain.
- A stream that has deteriorated suddenly over days rather than gradually over years.
- Continuous dribbling without warning, or new leakage in someone who has been straining to pass urine for a long time.

How should I prepare?
Arrive with a comfortably full bladder — enough to pass at least around 150 mL, but not so full that it is painful. Both too little and too much distort the result. Bring a list of your medicines, since some affect the bladder and the outlet.
Does it hurt?
No. Nothing is inserted. You pass urine privately into a funnel and the machine does the rest, which is what makes it a reasonable first test in almost anyone.
My flow rate was low. Does that mean my prostate is blocking me?
Not necessarily, and this is the commonest misreading. A low flow means either the outlet is obstructed or the bladder muscle is underactive, and the flow test cannot tell which. If the answer will determine whether you have a procedure, a pressure-flow urodynamic study is what settles it. Check the voided volume on your printout first, too — under about 150 mL, the reading should not be relied on.
What is a normal flow rate?
In men, above about 15 mL/sec is reassuring and below about 10 mL/sec is likely significant, with a grey zone between that is read alongside everything else. Flow declines with age normally. In women, rates are normally higher, and the shape of the trace and the residual volume tend to matter more than the peak number.
