General urology covers the diagnosis and treatment of conditions affecting the urinary tract in both men and women — the kidneys, ureters, bladder, and urethra — as well as the male reproductive system. Symptoms That Should Not Wait for an Appointment

Most urological problems can be dealt with in clinic. A few cannot, and they are worth knowing before you need them:

• Complete inability to pass urine with a painful, full bladder — acute urinary retention, which needs a catheter the same day
• Sudden severe testicular pain, particularly with nausea or a testis sitting higher than usual — possible testicular torsion, a surgical emergency where hours matter
• Fever with flank or back pain, shaking chills, or vomiting — a possible obstructed, infected kidney
• An erection lasting more than four hours — priapism, which causes permanent damage if left untreated
• Visible blood in the urine — not always urgent, but never something to watch and hope about, even if it clears on its own

Any of these means a hospital emergency department, not a clinic booking and not a message.

What Is Not Performed Here

Being clear about scope saves patients a wasted journey. HoLEP, PCNL, bladder Botox, sacral neuromodulation, mid-urethral sling and urethral bulking agents are not performed at Bangkok Hospital Headquarters, and TESA and PESA needle aspiration are not used. HIV testing, PrEP and PEP belong to the Infectious Diseases department, not to urology. Where one of these is genuinely the right answer for you, it is said plainly and a referral is arranged.

Q2: When should I see a urologist?

See a urologist for blood in the urine, recurrent urinary infections, difficulty or pain with urination, a weak stream or incomplete emptying, kidney or flank pain, an abnormal PSA result, or any persistent change in urinary or sexual function. Early evaluation often prevents small problems from becoming serious ones.

Q3: Which urological symptoms are emergencies?

Complete inability to pass urine with a painful bladder, sudden severe testicular pain (possible torsion, where hours matter), fever with flank pain or shaking chills, and an erection lasting more than four hours all need a hospital emergency department rather than a clinic appointment. Visible blood in the urine is not always urgent but always needs assessment, even if it clears by itself.

Q4: How are kidney stones treated?

Small stones often pass with fluids and medication, while larger or obstructing stones may need 3rd-generation ESWL (non-invasive shockwave) or flexible ureteroscopy with Holmium laser. A metabolic evaluation helps identify why stones form so a prevention plan can reduce future recurrence.

Q5: Do I always need antibiotics for a urinary tract infection?

Not always. A symptomatic UTI usually needs targeted antibiotics, but bacteria found in the urine without symptoms (asymptomatic bacteriuria) often should not be treated, as unnecessary antibiotics cause more harm than good. Correct diagnosis is key to avoiding both under- and over-treatment.
