Low-Intensity Extracorporeal Shock Wave Therapy (LI-ESWT) is a promising treatment for erectile dysfunction (ED). ED is an increasingly common complaint at my andrology office, affecting men across all age groups. I have previously discussed the 4 major causes of ED et oral tablet therapy for ED.

Before LI-ESWT became available, patients who failed oral ED medication (PDE5 inhibitors) were advised to pursue more invasive options:
- Intracorporeal injection therapy — self-injection directly into the penis before every sexual activity
- Penile prosthesis implant surgery — a major operation requiring a highly experienced surgical team

The first official publication on LI-ESWT for ED appeared in 2010. Since then it has proven to fulfill key clinical requirements: scientifically significant effectiveness in ED treatment, applicability even for patients who have failed oral therapy, day-care minimally invasive procedure, and no serious side effects when performed by a well-trained andrologist.

- Scientifically confirmed synergistic effect when combined with oral PDE5 inhibitor therapy

Limitations of LI-ESWT
- Maintenance sessions required over time:
- 10% of patients report ED recurrence at 3 months post-treatment
- Over 40% report ED recurrence at 2 years
- The maintenance program is individualized for each patient
- Spinal cord injury patients will not benefit from LI-ESWT, as the brain-to-penile nerve connection is permanently disrupted
- Untreated low testosterone patients require Testosterone Replacement Therapy alongside LI-ESWT, as hormonal deficiency is one of the 4 major causes of ED

Results are significant but require maintenance: approximately 10% of patients experience ED recurrence at 3 months and over 40% at 2 years post-treatment. A personalized maintenance schedule is planned with the andrologist. When combined with oral PDE5 inhibitors during the treatment period, there is a scientifically confirmed synergistic benefit. LI-ESWT is a day-care procedure with no serious side effects when performed by a trained specialist.
