Genital warts are one of the commonest complaints I see in the urology clinic. They are caused by human papillomavirus (HPV), and the distress they cause is mostly cosmetic — though that distress is real, and they can be stubborn to clear.

Important — please read before anything else. The instructions in this article are aftercare guidance for patients who have already been examined by a doctor and prescribed 25% podophyllin solution, and shown how to use it. Podophyllin is a caustic agent: it works by burning tissue, its strength varies between batches, and it can be absorbed into the body if too much is applied. Do not buy podophyllin and treat yourself with it. Genital lesions also need to be examined before treatment, because not everything that looks like a wart is one — and a lesion that is not a wart will not respond, or may need entirely different management.

The reassuring point to make first is that genital warts are benign. It is very rare for them to turn malignant, so the concern is almost entirely cosmetic and psychological. This article deals specifically with small lesions.

Small genital warts can be removed with 25% podophyllin solution. It works by chemical burn, flattening the raised HPV-related skin lesion.

Instructions for prescribed patients: applying 25% podophyllin to small genital warts

Follow these steps exactly. Incorrect application can cause a severe skin burn. If your doctor gave you different instructions, follow theirs.

  1. Wash the genital area with liquid soap and water.
  2. Dry the area thoroughly with a towel or a dryer.
  3. Identify each of the small warts to be treated.
  4. Apply petroleum jelly over the skin surrounding each wart. This is what protects the normal skin from being burned — do not skip it.
  5. Soak a cotton bud in the 25% podophyllin solution and place a single drop on each wart. One drop only.
  6. Check that every wart has been treated.
  7. With a clean cotton bud, wipe away any solution that has run onto surrounding skin.
  8. Leave the solution in place for 4 hours, then wash the area gently with water. Do not leave it on longer.
  9. Apply no more than once a week. Repeated applications can be continued until the result is satisfactory, but the weekly limit must be respected.
  10. Even once the warts have gone, keep examining yourself regularly — recurrence is common.

Stop and contact your doctor if you develop severe pain, blistering, an open sore or spreading redness of the surrounding skin, or if you feel unwell after an application.

Genital warts are a cosmetic problem that carries a real psychological weight for the people who have them. Podophyllin is one way of clearing small lesions, but it asks for patience and discipline from the patient.
