If you’ve ever had a urinary tract infection — that unmistakable burning, the constant urge to go, the cloudy or strong-smelling urine — you know how miserable it can be. And if you’ve had more than one, you may be wondering: why does this keep happening to me? Am I doing something wrong? The short answer is no, you’re not. The reasons UTIs come back are rooted in some genuinely fascinating biology, and understanding them is the first step to taking back control.

What Is a Urinary Tract Infection?

A urinary tract infection — commonly called a “bladder infection” — happens when bacteria (most often a type called E. coli) enter the urinary tract and take hold. The telltale symptoms are a burning feeling when you urinate, needing to go frequently or urgently, cloudy or strong-smelling urine, and sometimes lower belly pain. UTIs are very common, especially in women — a large share of women will have at least one in their lifetime. How Your Bladder Normally Protects Itself

Here’s something most people never realize: your bladder is remarkably good at defending itself, with several clever defenses working together. A protective mucus layer lines the inside of the bladder like a shield, blocking bacteria from sticking to the wall. The lining also produces natural germ-fighting substances that kill bacteria on contact. A special protein in your urine acts like a decoy, tricking bacteria into latching onto it instead of the bladder wall so they get flushed away. Regular urination physically washes bacteria out before they can settle in. And immune cells stand guard beneath the lining to catch anything that slips past. Together, these explain why most people — even those who don’t drink much water — rarely get UTIs. A healthy bladder is a fortress.

So Why Do UTIs Keep Coming Back?

If your bladder is so well defended, why do some people get infection after infection? This is called a “recurrent UTI,” and there are four main reasons it happens — and notably, most of them have nothing to do with hygiene or anything you did wrong.

What You Can Do to Help Prevent UTIs

While no approach prevents every UTI, these steps can meaningfully reduce your risk:

When to See Your Doctor

Reach out to your healthcare provider if you have burning or pain when urinating; a frequent or urgent need to go with little coming out; cloudy, dark, bloody, or foul-smelling urine; or pain and pressure in your lower belly or back. Most importantly, seek care promptly if you develop fever, chills, nausea, or vomiting — these can mean the infection has spread to the kidneys, which needs timely treatment.
