Proper aftercare matters as much as the operation. This page sets out the instructions given to Dr. Soarawee’s patients following sleeve technique circumcision. For a general overview of the operation itself, see circumcision: which method suits whom.
If you had your circumcision elsewhere, follow the instructions your own surgeon gave you. Techniques differ, and so do the timings that go with them. Use this page for general orientation and for the warning signs at the end, which apply whoever operated. This page describes aftercare for an adult. If you are caring for a child who has been circumcised, follow the instructions given for that child rather than these — the technique, the dressing, the wound care and the timings are all different in a child, and several of the sections below do not apply at all. The warning signs still do.

When to get help rather than wait
This section was missing from the earlier version of this page and belongs before the routine instructions rather than after them. Go back to the hospital the same day for any of the following. In an emergency in Thailand, call 1669.
- Bleeding that soaks through the dressing, or that keeps coming after firm pressure has been held for ten minutes.
- Inability to pass urine, or a swelling that makes passing urine difficult.
- Fever or shaking chills.
- Spreading redness up the shaft, increasing swelling after the first couple of days, or discharge with an offensive smell — as opposed to the ordinary swelling and bruising that settle steadily.
- The head of the penis turning dusky, dark or cold, or becoming numb beyond the first day. This is uncommon and needs looking at immediately.
- Pain that is getting worse after the first two or three days rather than easing.
Contact the hospital rather than any personal or messaging channel: Головной офис Бангкокской больницы, or Госпиталь Samitivej Sriracha on 088-022-1445. Outside working hours, go to the emergency department — post-operative bleeding is not something to wait on until a clinic opens.

The first six hours
- The penis will feel numb from the nerve block used during surgery — this is normal and expected.
- You can pass urine normally.
- Do not open or remove the pressure dressing. It is holding the wound against minor post-operative bleeding.
- You may gently open the pressure dressing the following morning. If anything looks unusual when you do, contact the hospital rather than waiting for your appointment.

The first week
- Complete all prescribed oral antibiotics and anti-inflammatory medication.
- Shower as normal, but keep the surgical wound away from water and soap to avoid irritation.
- Apply Terramycin antiseptic ointment to the wound twice daily after each shower.
- Morning erections may be uncomfortable as they stretch the healing tissue. This is normal and expected. Most men need nothing for it; if erections are causing significant pain, contact the hospital, since medication can be prescribed in specific cases.
- No sexual activity.
- No heavy exercise — no jogging, cycling or running.
- Walking is permitted.
- Light desk-based work is permitted; outdoor duties may be considered if wound care is maintained.
- Wear loose boxer-style underwear — avoid tight conventional underwear.

Weeks two to four
- Terramycin ointment is no longer needed — the skin has naturally sealed.
- Absorbable sutures will gradually shed over about a month. This is completely normal, and they do not need removing.
- Heavy exercise is permitted — increase the intensity gradually rather than all at once.
- Any type of underwear is acceptable.
- Still no sexual activity. The wound looks healed well before it is strong, and this is the point at which men most often come to grief. Patience here protects the result.
After four weeks
- The wound is healed and all activities are permitted without restriction.
- Residual firmness, mild swelling at the scar line and a slightly uneven appearance are common at this stage and continue to settle over the following months. The scar softens and fades for a good while yet, so judge the cosmetic result later rather than at four weeks.
- Reduced sensitivity of the head of the penis in the early weeks is common and usually settles as the skin adapts.

When can I resume exercise and sexual activity?
Walking from day one, and light desk work during the first week. Heavy exercise — jogging, cycling, running — resumes gradually from week two. Sexual activity is avoided until at least four weeks, even though the wound looks healed well before then, because the tissue is not yet strong enough. Following the timeline reduces the risk of wound problems and protects the cosmetic result.
Is pain from morning erections normal?
Yes. Morning erections stretch the healing tissue and commonly cause discomfort during the first week. It is a normal physiological event and usually needs no medication. If the pain is significant, contact the hospital — medication can be prescribed in specific cases. It typically settles over the first week or two as the wound heals.
How do I care for the wound in the first week?
Complete all prescribed antibiotics and anti-inflammatory medication. Shower normally but keep the wound away from direct water and soap. Apply Terramycin antiseptic ointment twice daily after showering until the end of week one. Wear loose boxer-style underwear. From week two the ointment is no longer needed, as the skin has sealed.
What is not normal, and what should I do about it?
Bleeding that soaks the dressing or continues after ten minutes of firm pressure, inability to pass urine, fever or chills, spreading redness or increasing swelling after the first couple of days, an offensive discharge, the head of the penis turning dark or cold, or pain that worsens rather than eases. The stitches are coming away. Is that a problem?
No — they are absorbable and shed gradually over about a month. They do not need removing. What does need reporting is the wound edge opening up, as distinct from a suture falling away.
