Vasectomy is a permanent method of male contraception, performed as a day case taking around an hour. The result depends as much on what happens afterwards as on the operation itself — particularly on not stopping other contraception too early.

If you had your vasectomy elsewhere, follow the instructions your own surgeon gave you. Use this page for orientation and for the warning signs below, which apply whoever operated.

When to get help rather than wait

This section was missing from the earlier version of this page and belongs before the routine instructions. Go back to the hospital the same day for any of the following. In an emergency in Thailand, call 1669.

What is normal in the first few days

Activity, week by week

  1. Week 1
    • Sedentary, desk-based activities only.
    • Avoid heavy exercise — jogging, cycling, running — because of the risk of dislodging the ligation material.
    • No sexual activity.
  2. After week 1
    • Heavy exercise is permitted.
    • Sexual activity is permitted — but another contraceptive method must still be used until success is confirmed by semen analysis.

Confirming it has worked

The pathology report confirms that both tubes were correctly divided. That is not the same as being sterile: sperm remain downstream of the cut and can still cause a pregnancy for some time afterwards. Clearing them is what the waiting period is for.

Once the test shows no sperm, the vasectomy is confirmed and other contraception can stop. Until then it cannot — and this is where most post-vasectomy pregnancies come from, not from the operation failing.

Two things worth knowing that were not on this page

It is not quite 100%

Vasectomy is among the most reliable forms of contraception there is. It is not absolute. Very occasionally the cut ends rejoin, and sperm reappear months or years after a clear test — rarely, but it happens, and it is the reason a pregnancy after vasectomy is investigated rather than dismissed as impossible.

Nothing about that argues against having one. It is stated here because a man is entitled to the true figure rather than a round number, and because it explains why a repeat test is arranged if there is ever any doubt.

A minority develop lasting scrotal ache

Most men have a few days of discomfort and nothing more. A small minority develop persistent scrotal or testicular ache lasting beyond three months — usually mild, occasionally troublesome, and treatable. It is a recognised outcome rather than a sign of a botched operation, and it belongs in the conversation before the procedure rather than after it.

What a vasectomy does not change

And it should be regarded as permanent. Reversal is technically possible and is neither reliable nor a reason to proceed on the basis that the decision can be undone. If there is any real doubt, that is a reason to wait rather than to book.

When can I resume exercise and sexual activity?

Sedentary activities only during week one, with no heavy exercise and no sexual activity. From week two both are permitted — but another contraceptive method must continue until semen analysis confirms success.

How do I know it worked?

By a semen analysis showing no sperm, performed at three months after the operation or after twenty ejaculations. The pathology report confirms both tubes were divided, which is not the same as being sterile — sperm remain downstream of the cut and can still cause pregnancy until they are cleared.

Is it 100% reliable?

Very nearly, and not absolutely. The cut ends very occasionally rejoin, so sperm can reappear even after a clear test. This is rare, it is not a reason to avoid vasectomy, and it is why a pregnancy afterwards is investigated rather than assumed impossible.

Will it affect my testosterone or my sex life?

No. Testosterone, erections, orgasm and sex drive are unaffected, and the semen looks unchanged since sperm form a tiny fraction of the volume. What it does not do is protect against sexually transmitted infection.

What is not normal afterwards?

Enlarging scrotal swelling, fever or chills, spreading redness or discharge, pain worsening after the first few days, bleeding that soaks the dressing, or inability to pass urine. Any of these means returning to hospital the same day. Bruising, mild ache and blood in the first few ejaculations are expected.
