Circumstances change. Men who chose a vasectomy in one chapter of life sometimes want children in the next, and a สำรวจสหรัฐฯ found that 2–6% of vasectomised men eventually ask about reversal. The question they all arrive with is the same one: will it actually work?

First, what a vasectomy did
Vasectomy is a day-care procedure for permanent male contraception. Under local anaesthesia the surgeon makes a small incision of about 1 cm in the scrotum, identifies both vas deferens, then divides, ties and cauterises them. The no-scalpel technique achieves the same thing through a puncture rather than an incision, with less pain and a shorter operating time.
Reversal is a much bigger operation than the vasectomy was
This surprises almost everyone. The vasectomy took twenty minutes under local anaesthetic; the reversal — vasovasostomy — needs general anaesthesia, several hours of operating time and an overnight stay. The two ends of the vas have to be rejoined under an operating microscope, and the quality of that microsurgery is one of the things that decides the outcome. It is not a procedure to have done by whoever is available.
Sometimes the simple rejoin is not possible. Where pressure has caused a secondary blockage in the epididymis, the surgeon has to perform a vasoepididymostomy instead — a considerably harder operation, joining the vas directly to the epididymis. That decision is often made during surgery rather than before it, so it is worth asking your surgeon in advance whether they perform both.
The two success rates, and why they differ
With microsurgical technique, 70–95% of men have sperm return to the ejaculate, while 30–75% of couples achieve a natural pregnancy. Read those two figures together, because the gap between them is the honest part of this conversation: getting sperm back is a surgical result, but having a baby depends on the couple, not the operation.
Time since the vasectomy
This is the single biggest factor on the surgical side. Reversal within 5 years gives the best results. Beyond 10–15 years the rates fall appreciably, largely because secondary epididymal blockage becomes more likely and the more demanding vasoepididymostomy is needed.
The female partner’s age and fertility
This factor is left out of most discussions of vasectomy reversal, and it should not be, because it often matters more than anything happening in the operating theatre. Female fertility declines with age, and a couple where the man had his vasectomy fifteen years ago is usually a couple where both partners are older now. The sensible approach is for both partners to be assessed before the decision is made, not for the man to have surgery and for the couple to discover the other half of the picture afterwards.
Surgical technique and the surgeon
Microsurgical technique in trained hands is what produces the figures quoted above. Ask how many of these your surgeon performs, and — importantly — whether they can convert to vasoepididymostomy if the anatomy demands it. After the operation
The result depends partly on how carefully these are followed — the join is delicate for the first several weeks.

- Desk work can resume after 3 days
- No sport or strenuous activity for 3 weeks

One expectation to set now: sperm do not reappear the week after surgery. The 1, 3 and 6-month analyses exist because the return is gradual, and a disappointing result at one month is not a failed operation.
If reversal is not the right route
Reversal is not the only path to a pregnancy after vasectomy. Sperm can instead be retrieved surgically from the testis and used for IVF with intracytoplasmic sperm injection. Dr. Soarawee performs testicular sperm extraction (TESE) and microdissection TESE (micro-TESE) — the latter using an operating microscope to search the testicular tissue for the areas most likely to contain sperm, and the technique of choice when sperm production itself is the problem rather than a blockage.
Which route makes more sense depends on the time since vasectomy, the female partner’s fertility, and how many children the couple hope for. Reversal has the advantage that it can lead to more than one pregnancy naturally, with no further procedures; surgical retrieval with IVF gives a per-cycle chance and avoids a long wait for sperm to return, but commits the couple to IVF.
