/>
The partner dynamic: why a penile implant is only half the story, and intimacy is a shared journey.

For decades, the conversation around treating stubborn erectile dysfunction (ED) — the kind that no longer responds to pills — has focused heavily on mechanics. As urologists, our success has traditionally been measured by technical questions: did the device work correctly? Was it firm enough? Did we avoid infection or mechanical failure? When a patient comes through surgery with a flawlessly working penile implant, it is tempting to assume the mission is fully accomplished.

But as any experienced sexual-medicine clinician will tell you, a firm erection is only one half of a far more complex equation. True intimacy does not happen in a vacuum — it happens within the shared space of a relationship.

Recently, at the American Urological Association (AUA) 2026 Annual Meeting in Washington, DC, I had the privilege of shadowing Professor Mohit Khera, one of the globally recognized leaders in men’s health. Among thousands of research abstracts, one presentation stopped me in my tracks: a study from the Mayo Clinic following couples for a full year after a penile implant. Listening to it was a reminder that the scalpel can restore anatomy — but communication and shared expectations are what ultimately restore a couple’s sexual well-being.

One thing before the rest: an implant that is becoming infected

This article is about the emotional and relational side of living with a device, which is where most of the real difficulty lies. There is one physical problem that must never be sat on, and it can appear months or even years after the operation rather than only in the first weeks.

If you have a penile implant and develop redness, swelling, increasing pain, warmth, fever, or any discharge from the scrotum or the scar — or if any part of the device starts to become visible through the skin — be seen the same day. In Thailand you can call 1669. An infected implant does not settle with antibiotics bought from a pharmacy: the bacteria sit on the surface of the device where antibiotics cannot reach them properly, and the usual answer is to remove or exchange the device. Delay is precisely what turns a salvageable situation into one where the implant has to come out. The same applies to pain that is getting worse rather than better after the early healing period, and to a device that becomes impossible to inflate or deflate.

A Closer Look at the Mayo Clinic Findings

The researchers tracked a large group of patients across several clinics, with surgeries performed by multiple experienced surgeons. Rather than taking a single after-surgery snapshot, they surveyed patients and their partners at three key points: before surgery, around six months afterward, and again at one year.

At first glance, the headline numbers were reassuring. Patient satisfaction with the device was high — and, encouragingly, it actually rose further between the six-month and one-year marks, suggesting the satisfaction is durable. The share of couples who were sexually active also climbed over the year. If we stopped reading there, we would give ourselves a well-deserved pat on the back.

Two things are worth noting about how to read this. Couples who agree to be followed for a year after implant surgery are, by definition, couples still engaged with the process, so the picture is likely a little rosier than the full population of implant patients. And satisfaction surveys measure how people feel about a decision they have already made. Neither point undermines the findings; both are reasons to treat them as a guide rather than a guarantee.

The real value of the study lies in its willingness to look past the surface and explore the hidden barriers — the specific obstacles that quietly keep a couple from fully enjoying a perfectly functioning device.

The Hidden Barriers Most People Never Expect

When the researchers asked sexually active patients what kept them from using their implant more often, a fascinating gap emerged between mechanical function and relationship dynamics.

The traditional mechanical worries — the device not being firm enough, or breaking down — stayed remarkably rare, even a full year later. That is a testament to the engineering of modern implants. Instead, the barrier that grew the most over time was a deeply human one: a lack of interest from the partner, which noticeably increased between the six-month and one-year marks.

This reveals something important. In the early honeymoon phase after surgery, the novelty and relief of restored function often carry a couple forward. But by the one-year mark, deeper relationship dynamics, mismatched levels of desire, and long-unaddressed anxieties tend to resurface. If a partner had already drifted away from intimacy after years of frustration with ED, placing a device in the patient does not automatically flip an emotional switch in the partner’s mind.

It is also worth saying that a partner’s reduced interest is not automatically a relationship problem, and treating it as one can make things worse. In a couple of this age, the partner may be dealing with vaginal dryness or discomfort after the menopause, with the fatigue of a chronic condition, with medication that lowers desire, or simply with years of habit built around not having sex. Some of those have straightforward treatments of their own. The unhelpful move is to assume the answer lies entirely in the man’s device or entirely in the partner’s feelings.

Awkwardness, Not Failure

The study dug deeper into the experience of intimacy itself. When patients were asked about their concerns during sex with the implant, the top worries were not about the device breaking, eroding, or causing harm — those fears were rare. Instead, the most common issues were experiential: difficulty with certain positions, a sense of awkwardness, and some discomfort — and these tended to grow modestly over the year rather than fade.

What does this tell us? While the implant provides reliable firmness, the experience of sex changes. An engineered erection does not behave exactly like a natural one — it changes the angle and pivot point, the flexibility of the shaft, and the overall feel during different positions. When a meaningful share of patients describe intimacy as awkward or limited by positioning, it is a clear sign that good aftercare cannot simply end with confirming the device inflates. We must actively guide couples through adjusting to a new sexual normal.

One distinction matters here, though. Awkwardness and mild discomfort while a couple adapts are expected and settle with time and guidance. Pain that is persistent, worsening, or new after a period of comfort is not part of adapting — that needs examining rather than tolerating, because it can be the first sign of a problem with the device or its position.

The Partner’s Perspective

Despite these hurdles, the study delivered a heartening finding about the partners themselves. When surveyed, the overwhelming majority expressed genuine gratitude and satisfaction with the procedure — and nearly all said they would recommend it to other couples facing similar struggles. That willingness to recommend it actually grew stronger over the course of the year.

This contrast is striking, and worth sitting with. A partner can be deeply supportive, satisfied with the outcome, and enthusiastic about recommending the procedure — while at the same time experiencing a personal dip in sexual interest or some awkwardness during intimacy. It reinforces a useful reality: a partner’s love and validation of the patient’s journey are independent of their own desire. Partners generally want this to succeed; they simply need help navigating the long-term reality of living with the device.

Beyond the Scalpel: Treating the Couple, Not Just the Patient

Listening to this presentation shifted how I look at this kind of surgery. It forced me to ask: are we doing enough to prepare the couple as a unit? If we only counsel the patient, we are treating only half of the picture.

1. Aligning Expectations Before Surgery

Long before any consent form is signed, there should be an open, honest conversation with both the patient and the partner about what an implant can and cannot do. That means frankly discussing the possibility of perceived length changes, differences in sensation, and the fact that an engineered erection has a different feel and pivot point than a natural one. It also means being clear that the operation cannot be undone: implanting the device permanently alters the erectile tissue, so natural erections and any response to tablets or injections will not return afterwards. The risks of infection and of needing revision surgery years later belong in the same conversation. Aligning expectations before surgery dramatically reduces the sense of surprise or disappointment that can surface later. You can read more about the operation itself on the penile prosthesis service page.

2. Sexual Rehabilitation and Counseling After Surgery

Care should not end when the surgical wound heals. Once the device is in use, couples benefit from practical guidance — for example, using plenty of lubrication (vaginal dryness is common in postmenopausal partners and can worsen positioning discomfort) and openly discussing adjustments to positioning. If a couple’s old favourite positions now feel awkward, they deserve both the permission and the clinical guidance to comfortably explore new ways of finding pleasure together. Where the partner’s own discomfort is the limiting factor, that deserves assessment and treatment in its own right rather than being worked around indefinitely.

3. Checking In Systematically Over Time

Structured check-ins at the six-month and one-year marks — screening for partner satisfaction, positioning discomfort, and intimacy barriers — let us identify struggling couples early and step in with targeted counseling, before they quietly stop using the device altogether. Those visits are also when device function, pain and any early sign of a problem get checked, which is the practical argument for keeping them even when everything feels fine.

True Intimacy Is a Shared Journey

Modern urologic surgery can do a great deal — bypassing damaged nerves, overcoming severe vascular disease, and reliably restoring the mechanics of an erection. But as this research reminds us, a successful operation is the foundation, not the finished structure. Sex is an intricate mix of vulnerability, confidence, physical sensation, and emotional connection.

When we treat erectile dysfunction, we are not just repairing an organ — we are restoring a bridge between two people. By including the partner’s perspective, addressing the natural anxieties of adjusting to something new, and expanding our focus from mechanical success to relational well-being, we can help couples move from a successful operation to a genuinely fulfilling life together.

Disclosure of industry relationship: Dr. Soarawee has received training and travel support from Boston Scientific, which manufactures penile implants. The overwhelming majority of partners report being satisfied with the outcome and would recommend the procedure to other couples. Modern implants are also reliable mechanically, with breakdowns and firmness problems being rare even a year after surgery. It is worth remembering that these figures come from couples who chose the operation and stayed in follow-up, so they describe a motivated group rather than everyone.

What are the most common challenges after getting a penile implant?

The biggest day-to-day challenges are usually not mechanical. The most common concerns reported by patients are experiential — such as awkwardness or difficulty with certain sexual positions — because an engineered erection feels and pivots differently than a natural one. Over the longer term, a partner’s level of sexual interest can also become a significant factor. These are normal, manageable issues that respond well to counseling and guidance. Separately, infection and the possibility of revision surgery are the medical risks discussed before the operation.

What signs after a penile implant need urgent attention?

Redness, swelling, warmth, increasing pain, fever, or any discharge from the scrotum or the scar need same-day assessment, as does any part of the device becoming visible through the skin. In Thailand you can call 1669. These can appear months or years after surgery, not only in the first weeks. An infected implant does not settle with antibiotics from a pharmacy, because bacteria sit on the surface of the device where antibiotics cannot reach them, and the usual answer is removal or exchange — delay is what makes that more likely. Pain that is worsening rather than settling after the early healing period, or a device that cannot be inflated or deflated, also needs prompt review.

Does a penile implant feel the same as a natural erection?

A penile implant provides reliable firmness sufficient for intercourse, but the experience is somewhat different from a natural erection. It can change the angle, pivot point, and flexibility of the shaft, which is why some couples notice awkwardness with certain positions at first. With realistic expectations set before surgery, adequate lubrication, and a willingness to explore comfortable new positions, most couples adapt well and report a fulfilling intimate life. Persistent or worsening pain, however, is not part of adapting and should be examined.

Should my partner be involved in the decision and recovery?

Yes, ideally. Intimacy is a shared experience, and research suggests that involving the partner improves outcomes. Including the partner in honest conversations before surgery helps align expectations, and involving them in recovery and sexual rehabilitation afterward helps the couple adjust together. Counseling both partners, rather than just the patient, is associated with greater long-term satisfaction and more consistent use of the implant.

Can a partner be supportive even if their own sexual interest has declined?

Yes, and this is very common. Research shows that a partner can be deeply supportive, highly satisfied with the surgical outcome, and enthusiastic about recommending it, while still experiencing a personal decline in their own sexual desire. A partner’s love and validation are independent of their libido. It is also worth checking whether something treatable sits behind the reduced interest — vaginal dryness or discomfort after the menopause, fatigue from a chronic condition, or medication that lowers desire are all common and all manageable. Recognizing this allows couples and their doctors to address desire and intimacy openly, rather than treating it as a failure of the surgery.

Can my partner and I discuss this together by video consultation?

Yes, and for this particular decision it works well. Bangkok Hospital Headquarters offers a Telemedicine service, arranged in advance by emailing the Urology department at bhquro@bdms.co.th. A video consultation lets both of you ask questions in your own home, which many couples find easier than a first clinic visit, and it is a sensible way to work through expectations before committing to anything. Examination and surgery require an in-person visit, and any sign of device infection needs same-day in-person assessment. Samitivej Sriracha Hospital is in-person only.

{
  "@context": "https://schema.org",
  "@graph": [
    {
      "@type": "MedicalWebPage",
      "@id": "https://drsoaraweeurology.com/2026/09/22/penile-implant-partner-intimacy-success/#webpage",
      "url": "https://drsoaraweeurology.com/2026/09/22/penile-implant-partner-intimacy-success/",
      "name": "The Partner Dynamic: Why a Penile Implant Is Only Half the Story",
      "headline": "The Partner Dynamic: Why a Penile Implant Is Only Half the Story",
      "description": "Why partner involvement, expectations and communication decide whether penile implant surgery leads to a satisfying intimate life, drawing on one-year couple follow-up data presented at AUA 2026, and the device warning signs that need same-day assessment.",
      "inLanguage": "en",
      "datePublished": "2026-09-22",
      "dateModified": "2026-09-02",
      "lastReviewed": "2026-09-02",
      "specialty": "https://schema.org/Urologic",
      "medicalAudience": { "@type": "MedicalAudience", "audienceType": "Patient" },
      "isPartOf": { "@id": "https://drsoaraweeurology.com/#website" },
      "author": { "@id": "https://drsoaraweeurology.com/#physician" },
      "reviewedBy": { "@id": "https://drsoaraweeurology.com/#physician" },
      "publisher": { "@id": "https://drsoaraweeurology.com/#physician" },
      "disambiguatingDescription": "Satisfaction figures come from couples who elected implant surgery and remained in follow-up, so they describe a motivated group rather than all implant patients. Pain that is worsening rather than settling after the early healing period, or a device that cannot be inflated or deflated, also needs prompt review."
          }
        },
        {
          "@type": "Question",
          "name": "What are the most common challenges after getting a penile implant?",
          "acceptedAnswer": {
            "@type": "Answer",
            "text": "The biggest day-to-day challenges are usually not mechanical. The most common concerns reported by patients are experiential, such as awkwardness or difficulty with certain sexual positions, because an engineered erection feels and pivots differently than a natural one. Over the longer term, a partner's level of sexual interest can also become a significant factor. These are normal, manageable issues that respond well to counseling. Separately, infection and the possibility of revision surgery are the medical risks discussed before the operation."
          }
        },
        {
          "@type": "Question",
          "name": "Does a penile implant feel the same as a natural erection?",
          "acceptedAnswer": {
            "@type": "Answer",
            "text": "A penile implant provides reliable firmness sufficient for intercourse, but the experience is somewhat different from a natural erection. It can change the angle, pivot point, and flexibility of the shaft, which is why some couples notice awkwardness with certain positions at first. With realistic expectations, adequate lubrication and a willingness to explore comfortable new positions, most couples adapt well. Persistent or worsening pain, however, is not part of adapting and should be examined."
          }
        },
        {
          "@type": "Question",
          "name": "Should my partner be involved in the decision and recovery?",
          "acceptedAnswer": {
            "@type": "Answer",
            "text": "Yes, ideally. Intimacy is a shared experience, and involving the partner improves outcomes. Including the partner in honest conversations before surgery helps align expectations, and involving them in recovery and sexual rehabilitation afterwards helps the couple adjust together. Counselling both partners rather than only the patient is associated with greater long-term satisfaction and more consistent use of the implant."
          }
        },
        {
          "@type": "Question",
          "name": "Can a partner be supportive even if their own sexual interest has declined?",
          "acceptedAnswer": {
            "@type": "Answer",
            "text": "Yes, and it is common. A partner can be deeply supportive, highly satisfied with the surgical outcome and enthusiastic about recommending it while still experiencing a personal decline in desire, because love and validation are independent of libido. It is also worth checking whether something treatable sits behind the reduced interest, such as vaginal dryness or discomfort after the menopause, fatigue from a chronic condition, or medication that lowers desire. Recognising this allows the couple and their doctors to address desire openly rather than treating it as a failure of the surgery."
          }
        },
        {
          "@type": "Question",
          "name": "Can my partner and I discuss this together by video consultation?",
          "acceptedAnswer": {
            "@type": "Answer",
            "text": "Yes.
