things men come to see me about. It rarely arrives suddenly. The symptoms build so gradually that most men adjust around them for years before mentioning it to anyone, which is a shame, because it is very treatable.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Symptoms that suggest BPH<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>A weak stream, and having to strain to get going<\/li>\n\n\n\n<li>The sense that the bladder never quite empties<\/li>\n\n\n\n<li>Getting up at night to pass urine, and waking tired because of it<\/li>\n\n\n\n<li>Passing urine more often through the day<\/li>\n\n\n\n<li>A sudden, hard-to-defer urge to go<\/li>\n\n\n\n<li>Dribbling at the end, or a stream that splits<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">None of these is dangerous in itself. Men with androgen insufficiency from birth do not develop BPH, which is what established the hormone&#8217;s role.<\/li>\n\n\n\n<li><strong>Age.<\/strong> The consistent finding across the literature is that ageing is the fundamental trigger. The gland gains both more cells and larger ones over time, which is why measuring prostate size is part of the assessment.<\/li>\n<\/ol>\n\n\n\n<p class=\"wp-block-paragraph\">Read those three together and the conclusion follows: <strong>an ageing man with normal hormones will develop some degree of BPH.<\/strong> US data bear that out \u2014 it affects 50% of men aged 50 and over, 70% of those aged 60\u201369, and more than 80% of men aged 70 and above. This is not a disease you catch or bring on yourself; it is closer to a feature of male ageing.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What you can actually influence<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">BPH cannot be prevented outright, since neither ageing nor androgen is optional. Its <em>pace<\/em> is another matter, and two risk factors are within your control:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li><strong>\u0e04\u0e27\u0e1a\u0e04\u0e38\u0e21\u0e42\u0e23\u0e04\u0e1b\u0e23\u0e30\u0e08\u0e33\u0e15\u0e31\u0e27\u0e43\u0e2b\u0e49\u0e14\u0e35<\/strong> \u2014 there is strong evidence that hypertension, diabetes and dyslipidaemia increase prostate size.<\/li>\n\n\n\n<li><strong>\u0e2b\u0e25\u0e35\u0e01\u0e40\u0e25\u0e35\u0e48\u0e22\u0e07\u0e20\u0e32\u0e27\u0e30\u0e2d\u0e49\u0e27\u0e19<\/strong> \u2014 which appears to raise oxidative stress within prostatic tissue and contribute to enlargement.<\/li>\n<\/ol>\n\n\n\n<p class=\"wp-block-paragraph\">Controlling blood pressure and blood sugar, losing excess weight and staying active will not shrink the prostate, but they slow the trajectory \u2014 and they happen to be the same measures that protect the heart and the erections, which makes them an easy recommendation.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Why it is worth treating rather than tolerating<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Left long enough, obstruction has consequences beyond inconvenience: <a href=\"https:\/\/drsoaraweeurology.com\/th\/2020\/07\/11\/%e0%b8%81%e0%b8%a3%e0%b8%b0%e0%b9%80%e0%b8%9e%e0%b8%b2%e0%b8%b0%e0%b8%9b%e0%b8%b1%e0%b8%aa%e0%b8%aa%e0%b8%b2%e0%b8%a7%e0%b8%b0%e0%b8%ad%e0%b8%b1%e0%b8%81%e0%b9%80%e0%b8%aa%e0%b8%9a\/\">\u0e01\u0e23\u0e30\u0e40\u0e1e\u0e32\u0e30\u0e1b\u0e31\u0e2a\u0e2a\u0e32\u0e27\u0e30\u0e2d\u0e31\u0e01\u0e40\u0e2a\u0e1a<\/a> from urine that never fully clears, bladder stones, a bladder muscle that thickens and eventually works poorly, and \u2014 in severe, long-standing cases \u2014 damage to the kidneys.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The one that brings men to the emergency department is <a href=\"https:\/\/drsoaraweeurology.com\/th\/2020\/10\/21\/urinary-retention-%e0%b8%9c%e0%b8%b9%e0%b9%89%e0%b8%8a%e0%b8%b2%e0%b8%a2\/\">\u0e20\u0e32\u0e27\u0e30\u0e1b\u0e31\u0e2a\u0e2a\u0e32\u0e27\u0e30\u0e04\u0e31\u0e48\u0e07\u0e40\u0e09\u0e35\u0e22\u0e1a\u0e1e\u0e25\u0e31\u0e19<\/a> \u2014 suddenly being unable to pass urine at all, with a painfully full bladder. BPH is its commonest cause, and treating the prostate properly reduces the risk of ending up there.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">One thing to be clear about<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>BPH is not prostate cancer, and it does not turn into prostate cancer.<\/strong> It is not a pre-cancerous condition. The two can coexist, and they cause overlapping symptoms, which is exactly why the assessment includes examination and a PSA discussion rather than assuming one or the other.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">\u0e17\u0e32\u0e07\u0e40\u0e25\u0e37\u0e2d\u0e01\u0e43\u0e19\u0e01\u0e32\u0e23\u0e23\u0e31\u0e01\u0e29\u0e32<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Mild symptoms that do not bother you may need nothing more than monitoring.
