over fifty develop some degree of prostate enlargement, and many of them adjust to the symptoms so gradually that they stop noticing. The International Prostate Symptom Score (IPSS) exists to make that visible: it converts a vague sense of getting up at night and not emptying properly into a number, which is what allows a urologist to decide between watching, medicating and operating — and to tell later whether anything actually improved.

Urology center Bangkok hospital Thailand Booking online 02-310-3009 bhquro@bdms.co.th

Samitivej Sriracha hospital Chonburi 088-022-1445

Before the Score: What Is Not BPH

The IPSS measures symptoms, not their cause, and this is where it misleads people most often. Identical scores are produced by an enlarged prostate, an overactive bladder, a urethral stricture, poorly controlled diabetes, a neurological condition, or simply drinking a great deal in the evening. A high score does not mean your prostate is the problem, and a normal score does not mean your prostate is healthy. Notably, the IPSS says nothing whatsoever about prostate cancer — early prostate cancer usually causes no symptoms at all, so a reassuring score is not a cancer screen and should never be treated as one.

Go to an emergency department, or call 1669 in Thailand, for:

Complete inability to pass urine with a painfully full bladder — acute retention

Fever and chills with urinary symptoms, feeling generally unwell

And arrange assessment rather than filling in a questionnaire if you have visible blood in the urine, unexplained weight loss or new bone pain, or symptoms that appeared suddenly rather than gradually — gradual onset over years is the pattern that fits benign enlargement.

<img data-recalc-dims="1" decoding="async" width="800" height="450" data-attachment-id="4790" data-permalink="https://drsoaraweeurology.com/ko/2022/03/04/international-prostate-symptom-score/portrait-asian-chinese-senior-man-feeling-happy-smiling-at-home-older-male-relax-toothy-smile-looking-to-camera-while-lying-in-the-garden-at-home-in-the-morning-concept/" data-orig-file="https://i0.wp.com/drsoaraweeurology.com/wp-content/uploads/2022/03/Old-man.jpg?fit=1200%2C675&ssl=1" data-orig-size="1200,675" data-comments-opened="1" data-image-meta="{"aperture":"0","credit":"","camera":"","caption":"Portrait Asian Chinese senior man feeling happy smiling at home. Publishing the item wording on a public web page requires permission, which this site does not hold, so what follows describes what each question covers rather than reproducing it. Your doctor will give you the actual form — which is better anyway, since a previous version of this page carried a loose translation of the answer options that changed their meaning.

All seven questions ask about the last month. Six of them are answered on a frequency scale running from not at all through less than one time in five, less than half the time, about half the time and more than half the time to almost always, scored 0 to 5. The seventh, on night-time waking, is scored by the number of times, from none up to five or more — also 0 to 5.

Incomplete emptying — how often it feels as though the bladder has not emptied fully after finishing

Frequency — how often you have had to go again within two hours

Intermittency — how often the stream has stopped and started

Urgency — how often it has been difficult to postpone going

Weak stream — how often the flow has been weak

Straining — how often you have had to push or strain to begin

Nocturia — how many times you typically get up at night to urinate

Those seven split into two groups that behave differently and respond to different treatment: voiding symptoms (weak stream, straining, intermittency, incomplete emptying), which are what an obstructing prostate produces, and storage symptoms (frequency, urgency, nocturia), which come from how the bladder has adapted. A man whose score is driven almost entirely by storage symptoms may not be helped much by treating the prostate alone — which is one reason the total on its own is a blunt instrument.

How the Score Is Banded

The seven answers add to a total from 0 to 35:

Total score

Symptom severity

0–7

Mild

8–19

Moderate

20–35

Severe

Broadly, mild symptoms are watched with lifestyle measures, moderate symptoms usually prompt a discussion about medication, and severe symptoms bring procedures into the conversation. A change of around three points is generally regarded as the smallest difference a man actually notices, which is the figure worth remembering when judging whether a treatment has worked.

<img data-recalc-dims="1" decoding="async" width="800" height="533" data-attachment-id="4791" data-permalink="https://drsoaraweeurology.com/ko/2022/03/04/international-prostate-symptom-score/happy-senior-couple-having-a-good-time-at-park/" data-orig-file="https://i0.wp.com/drsoaraweeurology.com/wp-content/uploads/2022/03/Quality-of-life.jpg?fit=1200%2C800&ssl=1" data-orig-size="1200,800" data-comments-opened="1" data-image-meta="{"aperture":"2","credit":"","camera":"NIKON D750","caption":"Happy Asian senior couple having a good time. Two men can both score 14. One is mildly irritated and would rather not take a tablet every day; the other has stopped travelling, stopped drinking anything after six o’clock and is exhausted from broken sleep. The symptom score cannot tell those two men apart. The quality of life question can, and it is usually what decides whether treatment is worth it — so answer it honestly rather than stoically.

One more thing worth knowing, because it surprises almost everyone: prostate size correlates poorly with symptoms. A large prostate can cause very little trouble and a modest one can cause a great deal, depending on where the tissue sits and how the bladder has responded. That is why treatment follows the score and the quality of life answer rather than a measurement on a scan. What the assessment adds alongside the questionnaire is a flow rate, a check of how much urine is left behind after voiding, a urine test, PSA where appropriate, and a review of your medications — several common drugs make these symptoms worse.

Frequently Asked Questions

What is the International Prostate Symptom Score?

A validated seven-question instrument, also known as the AUA Symptom Index, assessing lower urinary tract symptoms over the past month: incomplete emptying, frequency, intermittency, urgency, weak stream, straining and nocturia. Each is scored 0 to 5, giving a total from 0 to 35, banded as mild, moderate or severe. A separate eighth question on quality of life is scored 0 to 6 and kept apart from the total. It is a copyrighted instrument, so the item wording is not reproduced here.

How is the IPSS interpreted?

A total of 0 to 7 is mild, 8 to 19 moderate, and 20 to 35 severe. Mild symptoms are generally watched with lifestyle measures, moderate symptoms prompt a discussion about medication, and severe symptoms bring procedures into consideration. A change of about three points is the smallest difference most men actually notice, so that is the benchmark for judging whether treatment has helped.

Does a high IPSS mean my prostate is enlarged?

Not necessarily. The same symptoms are produced by an overactive bladder, a urethral stricture, poorly controlled diabetes, neurological conditions and evening fluid habits. Prostate size also correlates poorly with symptoms: a large prostate may cause little trouble and a modest one a great deal. The score measures how much you are affected, not what is causing it, which is why assessment includes a flow rate, post-void residual, urine test and medication review.

Does the IPSS screen for prostate cancer?

No, and this is the most important misunderstanding to correct. Early prostate cancer usually causes no urinary symptoms at all, so a low IPSS provides no reassurance about cancer whatsoever. Cancer risk is assessed separately, through PSA testing and examination where appropriate, and that is a distinct conversation from symptom management.

Why is the quality of life question scored separately?

Because how much the symptoms bother you is a different question from how many symptoms you have, and it often matters more. Two men with identical totals can be affected completely differently — one mildly irritated, the other no longer travelling and chronically short of sleep. That answer usually decides whether treatment is worthwhile, so it should be answered honestly rather than stoically.

When do urinary symptoms need urgent attention?

Complete inability to pass urine with a painfully full bladder, or fever and chills with urinary symptoms, mean an emergency department or 1669 in Thailand. Visible blood in the urine, unexplained weight loss, new bone pain, or symptoms that appeared suddenly rather than gradually should be assessed rather than scored on a questionnaire.

Book an Appointment

Dr. Soarawee Weerasopone assesses and treats prostate enlargement and lower urinary tract symptoms at Bangkok Hospital Headquarters and at Samitivej Sriracha Hospital, Chonburi — Urology department 088-022-1445.
