cystoscopy is a look inside the bladder using a scope about the width of a drinking straw, done while you are awake, in a clinic room, in a few minutes. It is one of the most frequently performed procedures in urology and one of the most dreaded in prospect \u2014 usually far more than it deserves.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Rigid instruments came first, in 1878, and could not bend to follow the curve of the urethra, which made them genuinely painful in an awake patient. Blood that you can see needs the bladder inspected, because scans do not reliably show a flat tumour on the bladder lining. A normal ultrasound or CT is not a substitute for looking. This holds even if the bleeding happened once and stopped, and even if it was painless \u2014 painless is more concerning, not less.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The other reasons:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Surveillance after bladder cancer.<\/strong> Bladder tumours recur, so anyone treated for one has repeat inspections on a schedule for years. This is the commonest reason people have the procedure more than once.<\/li>\n\n\n\n<li><strong>Persistent irritative symptoms with negative cultures<\/strong> \u2014 urgency, frequency and discomfort where nothing grows. Carcinoma in situ can present exactly this way, which is discussed under <a href=\"https:\/\/drsoaraweeurology.com\/2022\/07\/07\/sterile-pyuria-general-ideas\/\">white cells in the urine with a negative culture<\/a>.<\/li>\n\n\n\n<li><strong>Assessing a poor stream<\/strong> where a <a href=\"https:\/\/drsoaraweeurology.com\/2021\/05\/14\/urethral-stricture-narrow-urethra\/\">urethral stricture<\/a> or bladder neck narrowing is suspected \u2014 often alongside a <a href=\"https:\/\/drsoaraweeurology.com\/2023\/04\/23\/uroflowmetry\/\">flow test<\/a>.<\/li>\n\n\n\n<li><strong>Removing a ureteric stent<\/strong>, which takes moments and avoids a second anaesthetic.<\/li>\n\n\n\n<li><strong>Investigating a suspected fistula or an anatomical abnormality.<\/strong><\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">What actually happens<\/h2>\n\n\n\n<ol class=\"wp-block-list\">\n<li>You lie on your back. Eat and drink normally beforehand \u2014 there is no fasting and no bowel preparation.<\/li>\n\n\n\n<li>The area is cleaned, and anaesthetic gel is put into the urethra and left for several minutes to work. This waiting time is not padding; going too early is the main reason the procedure is uncomfortable.<\/li>\n\n\n\n<li>The scope is passed gently while the urologist watches the screen, through the urethra and prostate into the bladder, which is filled with sterile fluid so the lining can be seen.<\/li>\n\n\n\n<li>The whole bladder surface is inspected systematically and the findings recorded. You can usually watch on the screen if you want to, and many people prefer to.<\/li>\n\n\n\n<li>It typically takes a few minutes. You go home straight afterwards and can drive yourself.<\/li>\n<\/ol>\n\n\n\n<p class=\"wp-block-paragraph\">Most people describe a strong urge to pass urine and a stinging sensation as the scope goes through, rather than pain. It is over quickly. Whether you are given them depends on your own circumstances \u2014 a urinary tract known to be colonised, a long-term catheter, immunosuppression, diabetes, a recent infection, or a procedure that turned out to be more than a simple inspection. Where they are used, a single dose around the time of the procedure is usually what is needed rather than a course afterwards.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This matters beyond one procedure. Antibiotics given to people who do not need them cause side effects, disturb normal bowel bacteria and select for resistant organisms \u2014 the same argument made elsewhere on this site about <a href=\"https:\/\/drsoaraweeurology.com\/2022\/07\/22\/asymptomatic-bacteriuria\/\">bacteria in the urine without symptoms<\/a>. It would be inconsistent to argue that there and then hand out a week of antibiotics after a five-minute inspection.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Blood thinners are also an individual decision<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The earlier version implied that anticoagulants and antiplatelet drugs would be held before the procedure as a matter of course. <strong>That is also decided case by case rather than by a rule.<\/strong> Diagnostic flexible cystoscopy carries a low bleeding risk, so for many people the medication simply continues. Against that sits the risk of stopping it \u2014 clot, stroke or a cardiac event \u2014 which for some patients is the greater of the two dangers. Which way it goes depends on why you take the drug, which drug it is, and whether anything beyond a plain inspection is planned.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Still tell your urologist exactly what you take, because the answer changes if a biopsy or any treatment is planned during the same procedure. But <strong>do not stop blood-thinning medication on your own initiative before a cystoscopy appointment<\/strong> \u2014 that decision belongs to the doctor who prescribed it.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Afterwards, and what is normal<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Stinging on passing urine for a day or two<\/strong>, and needing to go more often.
