comes up in clinic constantly: <em>what is the single best antibiotic for a bladder infection?<\/em> There isn&#8217;t one \u2014 different bacteria respond to different drugs. Those two positions cannot both be held.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">What current practice actually does is the opposite, and for exactly the reason the old article gave. <strong>An otherwise healthy, non-pregnant woman with straightforward cystitis is treated empirically with a short course of a narrow, bladder-specific antibiotic, and does not need a culture at all.<\/strong> The organism is predictable, the drugs used first-line are chosen precisely because they are not used for anything else, and a short course cures it.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Reaching for a broad-spectrum drug in that situation is not being thorough. It is genuinely necessary in these situations rather than in all of them:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Any man.<\/strong> See below \u2014 this is the important one on this site.<\/li>\n\n\n\n<li><strong>Pregnancy.<\/strong><\/li>\n\n\n\n<li><strong>Symptoms that have not settled<\/strong> on a first course, or that come back within a few weeks.<\/li>\n\n\n\n<li><strong>Recurrent infections<\/strong> \u2014 two in six months or three in a year.<\/li>\n\n\n\n<li><strong>Fever, flank pain, or feeling systemically unwell<\/strong>, which means it is no longer confined to the bladder.<\/li>\n\n\n\n<li><strong>A catheter, a stone, a stent, diabetes, immunosuppression, or a known abnormality of the urinary tract.<\/strong><\/li>\n\n\n\n<li><strong>Recent hospital admission, recent antibiotics, or recent travel<\/strong> \u2014 all raise the chance of a resistant organism.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">In those cases the culture is taken first, treatment is started, and the antibiotic is <strong>narrowed<\/strong> once the result arrives. Narrowing is the step that is most often skipped \u2014 a patient who is feeling better rarely gets called back to have the drug changed, and that is where a good deal of unnecessary broad-spectrum exposure comes from.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">A bladder infection in a man is not an uncomplicated one<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">This was absent from the earlier version, on a site read mainly by men.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The male urethra is long and urinary infection is correspondingly uncommon, so when it happens it is treated as a reason to ask why \u2014 <strong>incomplete emptying from an enlarged prostate, a stone, a stricture, or prostatitis<\/strong>. It also usually needs a longer course than the short one used in women, because the prostate is difficult for many antibiotics to penetrate.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A man treated with a three-day course and sent away, with nobody asking why it happened, has had half a consultation. It is also the reason a subsequent culture can come back falsely negative, which makes the real problem harder to identify.<\/li>\n\n\n\n<li><strong>Treating bacteria that are not causing symptoms.<\/strong> A positive urine culture in someone who feels perfectly well is usually <a href=\"https:\/\/drsoaraweeurology.com\/2022\/07\/22\/asymptomatic-bacteriuria\/\">not an infection to treat<\/a> \u2014 antibiotics there provide no benefit and select for resistance.
