stream is one of the hardest complaints to judge, because how a man feels about his flow and how his flow actually measures are only loosely related. Young men arrive convinced something is badly wrong and turn out to have entirely normal flows. Older men mention nothing, having adjusted over years, and measure poorly enough to be heading towards retention.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Uroflowmetry replaces the impression with a measurement. It is simple, takes no preparation beyond a full bladder, and involves nothing more than passing urine privately into a funnel. A great many men have been told they have a poor flow on the basis of a 70 mL void.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The opposite error exists too: a grossly overfull bladder that has been stretched produces a falsely low flow. So the instruction is to arrive with a <strong>comfortably full bladder, not a desperate one<\/strong>, and to say so if you could not manage a normal amount \u2014 the test can simply be repeated.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For the same reason, a single flow study is weak evidence. Flow varies between occasions in the same person, so two studies on different visits give a much fairer picture than one.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What the test measures<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Maximum flow rate (Qmax)<\/strong>, in millilitres per second \u2014 the headline number.<\/li>\n\n\n\n<li><strong>Average flow rate<\/strong>, which is often more representative than the peak.<\/li>\n\n\n\n<li><strong>Voided volume<\/strong> \u2014 the figure that determines whether the rest is valid.<\/li>\n\n\n\n<li><strong>Voiding time and time to peak flow.<\/strong> A stream that takes a long time to build up suggests the bladder is working hard to overcome something.<\/li>\n\n\n\n<li><strong>The shape of the trace<\/strong>, which a urologist reads as carefully as the numbers. A smooth bell shape, a long flat plateau, or an interrupted saw-tooth pattern point in quite different directions.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">It is almost always paired with an <strong>ultrasound scan of the bladder immediately afterwards<\/strong> to measure what is left behind. The flow rate and the residual volume together say considerably more than either alone.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Rough guides for the flow rate<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">These are conventions rather than diagnostic thresholds, and they assume an adequate voided volume.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>In men<\/strong>, a Qmax above about 15 mL\/sec is reassuring and below about 10 mL\/sec is likely to be significant. Between the two is a grey zone that is interpreted alongside symptoms, residual volume and prostate size rather than on its own. Flow also declines gradually with age in men with no disease at all.<\/li>\n\n\n\n<li><strong>In women<\/strong>, flow rates are normally considerably higher than in men. The earlier version of this article gave a normal range of 20 to 36 mL\/sec, which reads as though a faster flow were abnormal; it is not. class=\"wp-block-heading\">The limitation nobody mentions: a low flow does not tell you why<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">This is the most important thing to understand about the test, and the earlier version of this article did not say it.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A weak flow has two quite different explanations, and <strong>uroflowmetry alone cannot distinguish between them<\/strong>:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>The outlet is obstructed<\/strong> \u2014 an enlarged prostate or a urethral stricture \u2014 and the bladder is pushing hard against resistance.<\/li>\n\n\n\n<li><strong>The bladder itself is weak<\/strong> \u2014 an underactive detrusor \u2014 and is not generating enough pressure, with nothing blocking the way at all.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Both produce a low number on the same printout. Only a <strong>pressure-flow urodynamic study<\/strong>, which measures bladder pressure while you void, separates them reliably \u2014 and the distinction is not academic. <strong>Operating on the prostate of a man whose real problem is a weak bladder does not fix his symptoms<\/strong>, which is exactly why assessment before any prostate procedure matters more than the choice of technology. It tells you <em>that<\/em> the flow is poor, not <em>why<\/em>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">When it is used<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Assessing a weak stream in men<\/strong> \u2014 most often <a href=\"https:\/\/drsoaraweeurology.com\/2020\/08\/28\/benign-prostate-bph\/\">benign prostatic enlargement<\/a> or <a href=\"https:\/\/drsoaraweeurology.com\/2021\/05\/14\/urethral-stricture-narrow-urethra\/\">urethral stricture<\/a>, which the shape of the trace can help separate.<\/li>\n\n\n\n<li><strong>Voiding difficulty in women<\/strong>, including <a href=\"https:\/\/drsoaraweeurology.com\/2021\/12\/03\/female-underactive-bladder\/\">underactive bladder<\/a>.<\/li>\n\n\n\n<li><strong>Neurological conditions<\/strong> affecting bladder function.<\/li>\n\n\n\n<li><strong>Measuring the result of treatment<\/strong> \u2014 arguably its best use. Both too little and too much distort the result. Bring a list of your medicines, since some affect the bladder and the outlet.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Does it hurt?<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">No. Nothing is inserted. You pass urine privately into a funnel and the machine does the rest, which is what makes it a reasonable first test in almost anyone.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">My flow rate was low.
