\u201cP-shot\u201d \u2014 and the marketing is a good deal more confident than the evidence. This article sets out what PRP is, what the trials genuinely show, and why the major urological guidelines still place it in a different category from the treatments they recommend.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Availability, stated plainly.<\/strong> PRP injection <em>is<\/em> available at Bangkok Hospital Headquarters. Saying so does not change anything else in this article: it is offered as an <strong>investigational<\/strong> option, which means a man considering it should be told openly what the evidence does and does not show before he decides, rather than after. That conversation is the point of this page. But a plausible mechanism is not the same thing as a demonstrated benefit in patients, and this is precisely the gap that regenerative medicine marketing tends to step over. There is also a practical problem that follows from the method: PRP is not a standardised product. Platelet concentration, whether white cells are included, whether the platelets are activated and how many injections are given all differ between clinics, so two men receiving \u201cPRP\u201d may be receiving quite different preparations \u2014 and studies of one do not necessarily apply to another.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What the Evidence Actually Shows<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Here the honest summary is narrower than the marketing.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>The studies are small and short.<\/strong> The randomised evidence involves modest numbers of men followed for months rather than years. Improvements reported on questionnaire scores have generally been modest, and in placebo-controlled work the placebo groups also improve \u2014 which is why uncontrolled before-and-after results in this field are so unreliable.<\/li>\n\n\n\n<li><strong>Durability is unknown.<\/strong> There is no good long-term data on whether any benefit persists, or how often treatment would need repeating.<\/li>\n\n\n\n<li><strong>The major guidelines have not adopted it.<\/strong> Both the American Urological Association and the European Association of Urology continue to treat penile PRP as <strong>investigational<\/strong> \u2014 something that should be offered within a research protocol with proper consent, not as established care. That is a deliberate position, and it has not changed on the strength of the studies published so far.<\/li>\n\n\n\n<li><strong>Combination claims are weaker still.<\/strong> The idea that PRP plus shockwave therapy produces a dramatically better result than either alone rests on very limited data and should not be presented to patients as established.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">An earlier version of this article stated that PRP\u2019s benefit was clinically confirmed and that combining it with shockwave therapy produced dramatic improvement. That overstated what the evidence supports, and it has been corrected. Where the science lands in future, this page will follow it.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Safety: Better Than Most, But Not \u201cNo Risk\u201d<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Because PRP comes from the man\u2019s own blood, there is no risk of allergy or rejection to the material itself, and the reported safety profile in trials has been reassuring: bruising is common and settles on its own, and serious events have not been a feature of the published studies. But \u201cno serious complications in a few small short trials\u201d is a much weaker statement than \u201csafe\u201d, and any injection into the penis carries risks that belong on the page:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Bruising and haematoma \u2014 common, self-limiting<\/li>\n\n\n\n<li>Pain at the injection site<\/li>\n\n\n\n<li>Infection \u2014 uncommon, but any injection can introduce it<\/li>\n\n\n\n<li>Scarring or nodule formation at repeatedly injected sites, which is a theoretical concern with any repeated penile injection and is not something short trials would reliably detect<\/li>\n\n\n\n<li>The financial risk, which is not trivial: courses of regenerative therapy are usually paid out of pocket and are usually not covered<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Get help the same day if any of these happen after any penile injection<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>An erection lasting more than four hours<\/strong>, particularly a painful one occurring without arousal \u2014 this is priapism and is a true emergency; treatment within the first four to six hours usually resolves it, while delay beyond twenty-four hours makes permanent erectile dysfunction likely<\/li>\n\n\n\n<li><strong>Spreading redness, swelling, fever or discharge<\/strong> at the injection site<\/li>\n\n\n\n<li><strong>Bleeding that does not stop with steady pressure<\/strong>, or a rapidly enlarging swelling<\/li>\n\n\n\n<li><strong>Severe or worsening pain<\/strong> rather than the settling ache expected after an injection<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Attend an emergency department, or call <strong>1669<\/strong> in Thailand.
