/>
The plumbing report: how penile Doppler ultrasound measures blood flow to pinpoint the cause of ED.

If you have ever stood inside a packed convention center surrounded by thousands of the world’s leading surgical minds, you know the energy is electric. In May 2026, I had the privilege of traveling to Washington, DC, to attend the American Urological Association (AUA) 2026 Annual Meeting — not just as an attendee, but shadowing one of the true giants of modern men’s health, Professor Mohit Khera from the Scott Department of Urology at Baylor College of Medicine.

Working alongside Professor Khera is a masterclass in watching cutting-edge research translate directly into helping the man sitting across from you in the clinic. But one presentation in particular — from a talented team of Italian researchers in Milan — really made me stop and think. It tackled a quiet but important problem: the rigid, decades-old way we diagnose why a man’s erectile mechanics are failing. Their message was simple and powerful: if we want to accurately diagnose and fix a man’s circulation, we need a much better speedometer.

Erections Are an Engineering Problem

To understand why this research matters, it helps to think about erectile dysfunction (ED) not as a source of embarrassment or a comedy punchline, but as a pure engineering challenge. At its core, an erection is a plumbing event — a hydraulic system that depends entirely on good structure and strong flow.

When a man becomes aroused, the brain sends a signal that relaxes the muscle inside the penis — like opening a floodgate. Blood rushes in and fills two spongy chambers. As those chambers swell, they press against the outer wall and pinch the outgoing veins shut, locking the blood in place. It is a beautiful balance: high-volume inflow, and zero leak on the way out.

When this system breaks down, it usually happens in one of two ways:

Inflow problem (arteriogenic ED): The incoming pipes are narrowed, clogged, or stiff, so not enough blood can get in.

Outflow problem (venous leak): The trapdoor fails. Blood gets in fine, but the veins cannot pinch shut, so it escapes right back out.

Here is the crucial part. The arteries that supply the penis are extremely small — far smaller than the arteries feeding your heart or brain. That makes them the body’s ultimate early-warning system. If plaque or stiffness is starting to build up because of high blood pressure, high cholesterol, or early diabetes, it almost always shows up in these tiny penile vessels years before it ever causes a heart attack or stroke. That is why accurately diagnosing blood-flow-related ED is not just about restoring intimacy — it can genuinely save lives.

The Speedometer: Penile Doppler Ultrasound

When a man comes to an advanced men’s health clinic for erectile issues, we do not like to guess — we map the terrain. The best tool for this is a painless scan called a Penile Color Doppler Duplex Ultrasound, often shortened to a penile Doppler.

During the test, a small dose of medication is given to temporarily relax the muscle and mimic full arousal, then an ultrasound probe is placed gently on the skin to watch the blood flow in real time. On the screen, the blood appears in vivid reds and blues, and the machine measures exactly how fast it is rushing through those tiny channels at peak flow. Doctors call this the peak flow speed. Think of it like the speedometer reading for a man’s pelvic circulation.

At Bangkok Hospital Headquarters this is a team study. I give the injection myself and stay with the patient throughout, while the ultrasound imaging is carried out by a radiologist — so the urological interpretation and the imaging expertise come together in the same room.

For decades, urology guidelines have relied on a classic set of fixed cut-off speeds: above a certain reading meant healthy inflow, a middle band meant mild-to-moderate trouble, and below a certain line meant severe blockage. Clean, simple, objective. The trouble is, human biology rarely fits neatly into tidy little boxes.

The Problem With One-Size-Fits-All Numbers

The core issue the Milan team raised is that those traditional cut-off speeds were never rigorously tested against large, diverse groups of men sorted by how severe their ED actually is. In other words, the speed limits were inherited from old textbooks rather than proven on real-world patients.

Think of it like setting a single speed limit for every highway, mountain pass, and quiet residential street across an entire country — without ever checking how traffic actually flows on each one. Label a man as only mildly blocked based on a borderline reading, and you might completely miss the fact that his real situation is far worse. Or you might do the opposite and over-diagnose someone whose blood flow is actually fine.

To fix this, the researchers ran a careful real-world study. They did not only look at men with erectile problems — they also included a comparison group of men with completely normal erectile function, to create a true baseline. Then they used rigorous data analysis to ask: what should the speed limits really be?

What the Data Revealed

The findings were eye-opening. As expected, men with the most severe erectile dysfunction had dramatically slower peak blood flow than men with mild ED or healthy controls — the speeds dropped off sharply as severity increased. The relationship between slow flow and severe disease was crystal clear.

But the really useful part was this: when the researchers let the real-world data — rather than an old textbook — define the ideal cut-off for identifying severe arterial disease, it landed almost exactly on the long-standing traditional line. In other words, the classic threshold turned out to be remarkably accurate. What the new study added was a sharper, evidence-based understanding of the gray zone right around that border — the very place where men are most likely to be misjudged.

The Balancing Act: Missing Cases vs. False Alarms

Why does the exact cut-off matter so much to a real patient? Because where you draw the line creates a trade-off between two kinds of mistakes.

Draw the line too generously — flagging almost anyone with a slightly low reading — and you will catch nearly every man with a true blood-flow problem, but you will also raise a lot of false alarms. You might tell a man his arteries are severely blocked when his flow is actually decent and his real issue is psychological, hormonal, or nerve-related — subjecting him to needless anxiety or even unnecessary treatments.

Draw the line too strictly, and you gain great certainty for the men who fall below it — but you risk telling some men with genuine vascular disease that their blood flow is good enough, sending them home with their underlying heart risks completely unaddressed. The data-driven sweet spot threads this needle, validating that the traditional line is highly accurate while giving doctors a more refined tool for the borderline cases.

Why This Matters to You

Spend your days analyzing this kind of data under the mentorship of specialists like Professor Khera, and you realize that advanced medicine is often won or lost in exactly these gray areas. A single ultrasound number is never the whole story. A reading that looks acceptable on an old chart might, when cross-referenced with a man’s real symptoms, reveal an early cardiovascular warning sign that calls for prompt lifestyle changes, cholesterol management, or targeted therapy. Even higher flow readings can sometimes hide milder forms of arterial insufficiency that deserve a closer look.

If you are struggling with erectile health, here are the principles worth holding onto:

It is rarely just in your head. The large majority of long-standing erectile issues are rooted in the physical mechanics of blood flow.

Get a real roadmap. A prescription pill can temporarily mask the symptom, but advanced diagnostics like a penile Doppler ultrasound show exactly how your pelvic blood vessels are performing.

Look at the whole picture, not a single number. Every man’s anatomy is unique. A good specialist reads your ultrasound alongside your overall metabolic health, hormones, and lifestyle.

Attending AUA 2026 in Washington, DC, was a powerful reminder of how fast our field is evolving. By replacing outdated assumptions with robust, real-world data, we can move away from one-size-fits-all treatments and build precise, individualized plans that protect both a man’s sexual vitality and his long-term heart health.

If you are experiencing erectile difficulties and want a thorough evaluation that looks beyond a quick prescription, Dr. Soarawee Weerasopone offers specialist men’s health consultations at Bangkok Hospital Headquarters, where a penile Doppler study can be arranged together with the radiology department when it is clinically indicated. Book a Consultation. Consultations are also available at Samitivej Sriracha Hospital, Chonburi — please call the department line on 088-022-1445.

Bangkok Hospital Telemedicine is available for patients who cannot attend in person, including international patients — arrange it in advance by email to the Urology department at bhquro@bdms.co.th. A video consultation is a good way to review your history and decide whether imaging is needed, but the Doppler study itself always requires an in-person visit. Samitivej Sriracha is in-person only.

Frequently Asked Questions

What is a penile Doppler ultrasound and what does it show?

A penile Doppler ultrasound is a painless scan that measures how well blood flows into and stays within the penis during a state of full arousal. A small dose of medication is given to relax the muscle and mimic arousal, then an ultrasound probe shows the blood flow in real time. It helps doctors determine whether erectile dysfunction is caused by an inflow problem, an outflow (venous leak) problem, or another cause altogether, allowing for far more accurate, individualized treatment.

Who performs the penile Doppler study at Bangkok Hospital Headquarters?

It is done jointly. Dr. Soarawee gives the injection and remains with the patient throughout the study, while the ultrasound imaging is performed by a radiologist. The findings are then interpreted alongside the rest of the urological assessment, so the result is read in the context of your symptoms rather than as an isolated number.

Is erectile dysfunction usually physical or psychological?

While stress and anxiety can certainly contribute, the large majority of chronic erectile problems are rooted in the physical mechanics of blood flow rather than being purely psychological. This is especially true as men get older or develop conditions like high blood pressure, high cholesterol, or diabetes. A proper evaluation, sometimes including blood-flow imaging, can identify the true underlying cause instead of simply masking the symptom with a pill.

Why can erectile dysfunction be an early warning sign for heart disease?

The arteries supplying the penis are much smaller than those supplying the heart and brain. Because of their tiny size, they tend to show the effects of plaque buildup or vascular stiffness years earlier than larger arteries. This means blood-flow-related erectile dysfunction can act as an early warning of developing cardiovascular disease, giving men a valuable opportunity to address risk factors before a heart attack or stroke occurs.

What is the difference between arteriogenic ED and venous leak?

Arteriogenic ED is an inflow problem: the arteries are too narrowed or stiff to let enough blood into the penis. A venous leak is an outflow problem: blood enters normally, but the veins fail to seal shut, so it escapes before a firm erection can be maintained. These two causes require different treatment approaches, which is why an accurate diagnosis with tools like a penile Doppler ultrasound is so important.

Should treatment decisions be based on a single ultrasound number?

No. A single blood-flow reading should never be interpreted in isolation. The same number can mean different things for different men depending on their symptoms, age, hormones, and overall metabolic health. An experienced specialist evaluates the ultrasound results alongside the full clinical picture to reach an accurate diagnosis and build a personalized treatment plan, rather than relying on a one-size-fits-all cut-off.

Can I discuss this by video consultation before travelling?

Yes. Bangkok Hospital Headquarters offers a Telemedicine service, which can be arranged in advance by emailing the Urology department at bhquro@bdms.co.th. A video consultation is a sensible first step for reviewing your history, previous test results, and current medications, and for deciding whether blood-flow imaging is likely to add anything.
